Maxim Healthcare Staffing is hiring for a Case Worker who has experience assisting members with HIV or Trauma Brain Injury. This position is located in Chicago, IL and requires travel throughout Cook County.
They will work in collaboration with the multi-disciplinary team. They will have input in the development of the plan of care for those patients requiring social work intervention. Provides medically necessary intervention to patients who have social or emotional problems which are or are expected to be an impediment to the effective treatment of the patient's medical condition and/or rate of recovery.
Responsibilities:
- Visit members in the home to complete an assessment, develop a care/service plan that meets members needs and provide periodic follow-up
- Verifies eligibility for wavier services
- Documents findings in the Care Management System
- Meets or exceeds timelines associated with case activities
- Provides succinct presentation of member, and solidifies plan of care with interdisciplinary Care Team
- Completes a member assessment according to department policy and procedure.
Minimum Requirements:
- Must have either of the following: License Social Worker (LSW) in Illinois, OR Unlicensed Social worker with a Bachelors degree in Social Work, Social Sciences, or Counseling, OR Vocational Specialist plus Certified as a Rehabilitation Counselor (CRC) or three (3) years working with persons with disabilities
- Minimum of (2) year experience providing services and support to individuals with intellectual disabilities
- Minimum of one (1) year care coordination experience required
- Duties may involve travel to locations throughout the Cook County area, required
