We have an opening for a full-time case manager to assist seniors in the Peoria area to access services and resources in order to remain living in their own home. ACM Care is a local growing social service agency working with seniors (60+) in the community, hospitals, and nursing homes. A case manager assesses the senior's needs, determines eligibility for multiple programs, develops a person-centered plan of care, and provides referrals and support services.
Requires the ability to form working relationships with all of our local vendors, must have the ability to work in a fast-paced environment, and must have a valid Driver's License (travel throughout Peoria area will be required).

Job Summary: Responsible for providing case management services to frail elderly residing in the Peoria, Illinois area to assure the delivery of quality care and an optimal lifestyle to the client. Maintain online and PC-based database programs on case management clients. Participate in Outreach and Networking activities as needed.
