Job Objective: Case managers provide goal-oriented and individualized support focusing on improved self-sufficiency for the persons served through needs assessments, planning, linkage, advocacy, coordination, and monitoring activities.

- Provide on-going, goal-oriented support, counseling and case management expertise to beneficiaries
- Work with beneficiaries in support of individualized rehabilitation plans, complete (interim and follow-up) Self-Sufficiency Matrix, support Job Readiness, Career Portfolio Development, Employment/Housing Search and other self-sufficient and/or transitional related goals.
- Maintain and ensure that self-sufficient and transitional goals tracked on the rehabilitation plan address identified needs and are up-to date and documented weekly
- Continually monitor and evaluate each beneficiary’s progression through their case management goals, and develop corrective action revisions to the plan as needed
- Assist beneficiaries in obtaining necessary resources to achieve case management goals
- Collaboration, development, and connection of the beneficiaries to community sponsorships, mentors, and anonymous meetings.
- Utilization and connection of the beneficiaries to The Salvation Army resources and services including but not limited to the local Corps, Pathway of Hope, Harbor Light, etc.
- Maintain positive and mutually respectful relationships with other (non-TSA) human service providers, entitlement agencies, health care vendors, and the community
- Assist with center statistics and outcome measurements
- Maintain compliance, accuracy, and timeliness with documentation of service objectives and outcomes as well as other services in accordance with ARC Program guidelines
- Compliance with confidentiality policies and procedures
- Attend program-related community, coalition, and committee meetings as assigned
- Optimize resources and opportunities through community linkages and enhance social supports
- Actively participate in program-related meetings and trainings Performs other duties as assigned by supervisor
Additional Information
Minimum Qualifications:
Education & Experience:
- Bilingual
- Bachelor’s degree in human services or related field
- Minimum of 2 years combination of education/work related experience required
- Minimum of 2 years’ experience in social service setting with working knowledge of case management systems and planning techniques
- Knowledge of related resources and the Continuum of Care for disaffiliated persons
Skills/Abilities:
- Counseling experience
- Excellent verbal and written communication skills
- Computer proficiency in MS Office Suite
- Attention to detail
- Exceptional time-management skills
- Superior organizational skills
Supervisory Responsibility: No supervisory responsibilities.
Physical Requirements:
Speaking, hearing and vision ability, and excellent manual dexterity to be able to perform on computers. Lifting, pulling, and pushing of materials up to 50 pounds to be able to move equipment.
Travel: Minimal travel may be required for Command events and community partnerships.
