Overview: We are seeking a dedicated and compassionate individual to join our team as an FCS Case Manager. The successful candidate will be responsible for providing comprehensive case management services to clients, with a focus on fostering communication, building strong relationships, and facilitating access to resources and support systems. This position requires excellent communication skills, empathy, and the ability to work collaboratively with clients and other professionals.
Responsibilities:
- Conduct assessments to identify client needs, strengths, and goals.
- Develop and implement individualized service plans in collaboration with clients and other stakeholders.
- Provide ongoing support, advocacy, and guidance to clients in navigating social services systems.
- Coordinate and monitor services and referrals to ensure continuity of care.
- Facilitate communication and collaboration among clients, family members, service providers, and community resources.
- Cultivate and maintain relationships with community stakeholders, including local organizations, government agencies, and other relevant entities.
- Maintain accurate and timely documentation of client interactions, progress, and outcomes.
- Participate in conferences, team meetings, and trainings as required.
- Stay informed about relevant laws, regulations, and best practices in social services and case management.
- 65% percent of time will be spent working in the community.
- Managing a caseload of 18-25 clients.
Qualifications:
- Proven experience in case management or related social services roles.
- Excellent communication and interpersonal skills.
- Strong problem-solving and decision-making abilities.
- Ability to work effectively with individuals from diverse backgrounds.
- Knowledge of community resources and social services systems.
- Preferred accreditations in peer leadership, recovery coaching, or chemical dependency counseling.
- Lived experience is preferred.
- Commitment to ethical and professional standards in social work practice.
