HARBORVIEW’S MENTAL HEALTH AND ADDICTIONS SERVICES department has an outstanding opportunity for a MENTAL HEALTH OUTREACH CASE MANAGER
WORK SCHEDULE
100% FTE, 40 hours per week
Day Shift
POSITION HIGHLIGHTS
Promote community stability of patients by engaging them in MENTAL HEALTH services and treatment planning
Assist patients in securing and maintaining housing, benefits and treatment to meet their needs
Partner with agencies and other members of our patients’ support systems who are working with people in need of psychiatric assessment.
PRIMARY JOB RESPONSIBILITIES
Assist patients in their recovery from mental illness and substance use disorders
Assess patient needs and determine interventions
Assist clients obtaining housing and funding as necessary to prevent excessive use of crisis services
Facilitate diversion from voluntary and involuntary psychiatric hospitalization when clinically indicated
Participate in case reviews
Coordinate medication services with team’s medical staff
Prepare information requested by outside agencies such as DSHS, Social Security Administration, and Seattle Housing Authority
Testify in judicial proceedings in response to subpoenas
REQUIRED POSITION QUALIFICATIONS
A Master's Degree in behavioral sciences, such as psychology, psychiatric nursing, occupational therapy, vocational or educational counseling, or social work OR Bachelor's Degree in nursing or a behavioral science AND three years' of experience in the direct delivery of professional MENTAL HEALTH care.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/ registration.
Appropriate clinical licensure or registration in the State of Washington.
