### Responsibilities:
- Conduct comprehensive assessments of patients' medical, social, and psychological needs
- Develop individualized care plans based on assessment findings
- Collaborate with healthcare team members to coordinate patient care
- Monitor patients' progress and adjust care plans as needed
- Provide education and support to patients and their families regarding treatment options and resources
- Ensure compliance with HIPAA regulations and maintain patient confidentiality
- Facilitate communication between healthcare providers, insurance companies, and other stakeholders
- Assist with discharge planning and coordination of post-hospital care
### Experience:
- Previous experience in acute care or hospital setting is required
- Knowledge of coding systems such as ICD-10 for accurate documentation and billing purposes
- Familiarity with behavioral health principles and practices
- Understanding of discharge planning processes and resources available for post-hospital care
- Experience working with pediatric patients is preferred
- Strong knowledge of HIPAA regulations and ability to maintain patient confidentiality
- Excellent communication skills, both written and verbal
- Ability to work collaboratively in a multidisciplinary team
