Asociacion Mayab is a non-profit organization established in 2005. Rooted in Maya culture, we empower and respond to the needs of Maya families and individuals in San Francisco. We provide services that are year round, centered on health and healing and in collaboration with partners. Our programs include: COVID-19 education, outreach and community engagement; food pantry; translation services and language access educational workshops; information and referral; light case management; voting rights education; health and well-being workshops; and cultural activities and events. CASE COORDINATOR
Under the supervision of the Executive Director, the Case Coordinator is responsible for providing Maya families and individuals with appropriate referrals, technical assistance and support to navigate and overcome barriers, such as access to: food; housing; employment assistance; legal assistance; HSA (Medi-Cal, CalWORKS, CalFresh, General Assistance); DMV;
SFUSD; and other community and governmental resources. The Case Coordinator will conduct outreach, service connection, develop and implement family service plans, designed to promote self-sufficiency. They will also coordinate and support community building activities and events.
JOB DESCRIPTION/RESPONSIBILITIES:
The duties of the Case Coordinator include, but are not limited to:

● Develop outreach and partnership work plans with the Executive Director to prioritize families and individuals at risk of losing housing, lack access to food, recovering and healing from COVID-19, and are in need of legal support.
● Perform intakes and assessments to meet the needs of individuals and families.
● Provide direct assistance to families and individuals including but not limited to:
- Research, identify, assist and successfully link families in accessing direct social, community and health services
- Enrollment, navigation support and technology access
● Maintain participants’ files through regular progress notes and follow-up contact entries
● Attend all staff meetings as well as regular weekly team and case consultations meetings with partner organizations
● Meet with the Executive Director monthly to review progress.
● Stay in regular contact with partner organization Case Managers to streamline services for families and individuals.
● Write monthly reports and enter GMS data weekly.
● Provide culturally competent services (linguistically, cultural and generational).
● Provide interpretation services to families and individuals.
● Coordinate the Language Access, court/ medical translators ensuring assignments are connected.
● Co-create and coordinate the Language Access committee. This is committee will comprise of ten Maya community members to support developing more Maya translators
MINIMUM QUALIFICATIONS:
● Some college experience in the fields of social work, psychology, ethic studies, urban studies, and or law
● Minimum of 2 years of working with diverse underserved, vulnerable, systems involved families and individuals within the Maya and or Latinx communities
● Multicultural/Multilingual: fluent in speaking, writing and reading English and Spanish.
Preferred ability to speak Mam or Maya Yucateco
● Ability to work independently, as well as part of a team
● Working knowledge of indigenouse, Native and Latinx community and governmental service programs
● Strong written and verbal communication skills
● Experience with group facilitation and public speaking skills
● Strong computer skills and competence, including experience and ability to work with:
Window and MacOS operating systems; Microsoft Office (Word, Excel, Powerpoint); and Google (Gmail, Google Docs, Google Spreadsheets, Google Drive, etc.), virtual communication platforms like Zoom, Google Meets and Microsoft Teams
● Experience with and ability to document work with clients, including keeping up to date consent forms, intake forms, and case notes
● Ability to work some evenings and weekends
