The Care Coordinatorrole is a critical member of the Landmark Interdisciplinary Team. This role reports to Care Coordinator Supervisor, or Senior Operations Manager. Responsibilities are to ensure a positive patient experience with Landmark, as well as to build good relationships with local provider groups to ensure that Landmark is a reliable and trusted partner in patient care.
This position is full-time, Monday - Friday. Employees are required to work our normal business hours of 8:30am – 5:00pm EST. It may be necessary, given the business need, to work occasional overtime or weekends.
Our office is located at 2645 Meridian Parkway Durham, NC 27713. Employees will be required to work some days onsite and some days from home.
We offer up to 3 weeks of onsite training. The hours during training will be 8:30am to 5:00pm EST, Monday - Friday. After training is completed, will move into hybrid work model.
- All Telecommuters will be required to adhere to UnitedHealth Group’s Telecommuter Policy.
Primary Responsibilities:
- Regular contact with provider groups to collect clinical data.
- Support patients in scheduling home visits.
- Ensure that Landmark has all documentation available regarding patient conditions.
- Support Landmark providers and Nurse Care Managers with operational and clinical tasks.
- Handle escalated patient issues, ensuring the appropriate resources are involved.
- Follow triage protocol when needed.
- Other duties may be assigned.
- The position is Hybrid (will work between office and home)

Required Qualifications:
- High School Diploma / GED OR equivalent work experience
- Basic Proficiency in Microsoft office applications – Microsoft Excel (open, enter, edit, save data), Microsoft Word (opening, reading, saving files), and Outlook (email, calendar)
- Experience handling customer service calls (inbound and outbound)
- Must be 18 years of age OR older
- Ability to work our normal business hours of 8:30am to 5:00pm EST, Monday - Friday
Preferred Qualifications:
- Experience as a Medical Assistant, Pharmacy Tech, or other clinical or administrative role in a health care or office setting (i.e. handling electronic medical records, charting, scheduling tasks)
- Prior experience working with triage protocol
- Reside within commutable distance of 2645 Meridian Parkway Durham, NC
- Ability to keep all company sensitive documents secure (if applicable)
- Required to have a dedicated work area established that is separated from other living areas and provides information privacy
- Must live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
