Job Description
SUMMARY:
This is a professional-level position designed to serve individuals at risk of psychiatric institutionalization or hospitalization, individuals who are on discharge status in institutions or short-term facilities, individuals who have been admitted or discharged from state hospitals within the past two years, and individuals with multiple short-term inpatient admissions.

Essential Duties and Responsibilities:
The Duties and Responsibilities reflect the general details considered necessary to describe the principal functions of the job and shall not be construed as a detailed description of all related work assignments that may be inherent to the job.
- Coordinate services for all CRS individuals funded by the Managing Entity who have at least 2 admissions to crisis stabilization unit within a 6 mos. period.
- Assist above referenced individuals with appointment reminders, obtaining housing, employment and other activities that can aid in reducing readmission to crisis units/hospital.
- Manage, track and distribute incidental funding to above referenced individuals as approved by PPBHC administration in order to assist in recovery and reduce chance of readmission to crisis units/hospital.
- Coordinate care of above reference individuals with Crisis Unit Multidisciplinary Team.
- Assessment of high risk individuals in terms of current and potential strengths, weaknesses and needs.
- Develop holistic service plans with individuals addressing the broad needs characteristic of the service population.
- Link and refer individuals to all internal and external services needed.
- Ensure the proper documentation of progress notes related to requirements of Medicaid, licensing and accrediting agencies.
- Coordinate cases to facilitate implementation of the service plan.
- Manage case records to assure that proper documentation is contained and meets applicable standards.
- Initiate revisions and updates to reflect additions or changes in Medicaid, licensing and accrediting agencies.
- Monitor individual’s progress with respect to the provisions of the service/treatment plan.
- Evaluate impact of services on individual’s quality of life.
- Performs other duties as required by supervisor.
ADDITIONAL DUTIES:
- Advocate on the behalf of persons served to ensure that needs are met.
- Provide supportive crisis intervention to individuals when needed.
- Complete initial authorizations as required for new referrals.
- Monitor and follow up on authorizations needed to provide ongoing services.
- Review records to assure documentation meets applicable standards.
- Consult with Case Managers/CSU Treatment Team on difficult cases and provide a second opinion, expertise and problem solving assistance.
- Attend meetings with case management department and CSU Treatment Team for status of cases.
- Provide education and information to interested groups concerning available case management and community support services.

- Maturity and sensitivity to the special needs of PPBH individuals served while possessing the knowledge of behavioral health.
- Ability to assist individuals served in working with their case manager in determining steps needed to take in order to achieve goals.
- Ability to communicate individual’s needs and information to behavioral healthcare staff.
- Ability to utilize and navigate an electronic health record, Microsoft office software, and insurance verification websites.
- Must be service oriented with good customer service skills.
- Must possess the ability to maintain client confidentiality.
QUALIFICATIONS, EDUCATION AND/OR EXPERIENCE:

- Bachelor’s degree in a mental health or related field with at least 3 years of case mgt. experience.
- Ability to communicate effectively, both verbally and in writing.
- Knowledge of the local community resources desirable.
- Excellent organizational skills
- Ability to prioritize the needs of the clients.
- Able to work independently.
- COMPUTER SKILLS: Must have Intermediate to advanced experience in MS Office Suite (Outlook, Word, Excel, PowerPoint), the use of personal computers, and standard office productivity software.
- REASONING ABILITY:Ability to apply commons sense understanding to carry our instructions furnished in written, oral or diagram form.
REQUIREMENTS:
- A valid Class E Drivers’ License for positions that require driving personal or Agency vehicle on Agency business.
- Must maintain a minimum Bodily Injury & Property Liability insurance limit of at least $50,000 for positions that require driving personal own vehicle (POV) on Agency business. (Proof Required)
- PPBH is a 24/7 Agency. All employees must be available as needed when required.
