CHICAGO TEMPLE CORPS

POSITION DESCRIPTION

 

POSITION TITLE:      Bilingual Case Manager

LOCATION/DEPT:     Chicago Temple Corps

REPORTS TO:           Community Ministries Director

FLSA CATEGORY:    Non-Exempt

STATUS TYPE:         RPT

 

OUTCOMES

The Bilingual Case Manager is responsible for providing case management to Spanish speaking individuals and families at the Chicago Temple Corps. The Bilingual Case Manager works under the guidance of the Community Ministries Directors and the Corps Officers implementing the program’s objectives, such as, providing basic needs and/or emergency assistance services to individual and families, the enhanced services offered through Pathway of Hope, keeping required records, and linkage to community resources to support desired outcomes of participating individuals and families.

 

ESSENTIAL DUTIES AND RESPONSIBILITIES

Case Management (60%)

  1. Serves as the primary point of contact for Spanish speaking clients in helping them obtain basic assistance, services, resource/referral information and case management as needed within current policy guidelines.
  2. Develop/implement customized case management for interested and willing family units/participants in seeking to provide resources that extend and cultivate the holistic well-being of the entire family unit through the Pathway of Hope Program. Pathway of Hope duties include:
    1. Participate in cluster meetings led by the Regional Coordinators
    2. Participate in divisional or territorial educational forums or webinars designated to facilitate understanding of a wide range of topics so to enhance services with emergency assistance.
    3. Complete Territorial Caseworker certification program
    4. Perform time sensitive assessment screenings, goal assistance and collaboration with community resources to best help the family meet personalized goals.
    5. Meet weekly face-to-face or by phone with POH participants.
  3. Monitor/document participant contact and progress while properly maintaining records/files.
    1. Must enter client data and maintain reporting of information into The Salvation Army SIMS data system that supports local, territorial, and national impact and outcomes.
    2. Prepares monthly reports on Pathway of Hope progress and status.
    3. Maintains client confidentiality.
    4. Follows written policies and procedures to determine eligibility for services.
    5. Provides guidance and promotes the spiritual dimension of The Salvation Army social service program.
    6. Provide support during Food Pantry hours.

Administration (30%)

  1. Maintain spreadsheet with provided EA Assistance.
  2. Maintain a waitlist for EFSP rental and utility assistance.
  3. Maintain the client assessment binder.
  4. Review files to ensure they are in clear, and concise order.
  5. Gather data for monthly statistical reports.

Other Duties (10%)

  1. Provide Spanish translation support for corps social ministry activities: Back to School, Thanksgiving, Christmas, and Winter Coat Distributions.
  2. All other duties as assigned.

REPORTING RELATIONSHIPS

Under direct supervision of the Community Ministries Director, will work with director to provide case management support to non-English speaking clients.

 

This individual relates to, and interacts with, a wide range of contacts both within and outside The Salvation Army and in these contacts, they act as a representative of the Army and its mission.

 

PERFORMANCE MEASUREMENTS

This individual will be evaluated on how effectively the outcomes of this position are achieved, as well as the timeliness, accuracy, and completeness of accomplishing assigned goals.

 

A 30, 60, and 90-day performance review will be conducted.

 

Complete the Caseworker Certification Program within the first 90 days.

 

EDUCATION/ EXPERIENCE

  • Fluent in Spanish required. Professional written and verbal communication skills in Standard Spanish concisely articulating information for application, presentation and public speaking preferred. 
  • Bachelors’ degree in human service area required. On a case-by-case basis an associate degree plus experience in strength-based case management may substitute for bachelor’s degree.
  • Minimum of two years of experience in casework or social service.
  • A valid driver’s license and clearance of The Salvation Army’s MVR process.

  • Good communication (verbal and written) and interpersonal skills both in English and Spanish Languages.

  • Supportive of the mission of The Salvation Army.

POSITION LIMITATIONS

  • This individual will only commit Army resources that have been allocated or approved.
  • This individual will keep the Community Ministries Director informed on all critical issues relating to their area of responsibility.
  • This individual will adhere to all policies and procedures in carrying out the responsibilities of this position.

PHYSICAL DEMANDS/WORK ENVIRONMENT

  • This position is required to do light physical work. 
  • In order to successfully perform the essential functions of this position, the employee is regularly required to use standard office equipment and a PC, participate in community and outreach events/distributions, and make presentations to groups/individuals. 
  • The work environment for this position includes an office environment with a low to moderate noise level.
  • Maintain positive work atmosphere by behaving and communicating in a manner that fosters and maintains positive relationships with co-workers and supervisors.  This would include but not be limited to such actions: resolution of conflicts in a positive manner; courteous treatment of staff, clients, and visitors; respect of others’ property and person, professional and appropriate communication to and about co-workers, supervisors, and subordinates.  Support the Mission of The Salvation Army by treating every donor, contact and colleague with respect and dignity, and without discrimination of any kind.
