Job description

Purpose:

The purpose of the Case Manager position is to assist SASS and treatment teams in providing outreach, care coordination, aftercare services, and family/community support to consumers who receive applicable services. In a culturally aware and anti-racist manner, assist with supporting consumers in achieving recovery via their personal and familial goals and preferences, support in emotion regulation and symptom management, and assist in identification and use of natural and community supports and resources while articulating C4’s mission and vision.

Duties and Responsibilities:

  • Provide case management, outreach, care coordination, and aftercare services including, but not limited to: research and connection to internal and external services, psychoeducation and support in the community mental health system, navigation of human service and social support agencies, brief intervention and de-escalation approaches, and navigating level of care decisions related to psychiatric hospitalization and crisis assessment
  • Assess for indicators of abuse, neglect, and imminent risk
  • As appropriate, provide crisis intervention services to assess and stabilize individuals, with the goal of immediate symptom reduction, stabilization, and a return to a person-centered baseline level of functioning
  • As appropriate, provide brief and immediate mental health services or referral, linkage, and consultation with other mental health services
  • Provide culturally appropriate, competent, and individualized crisis, discharge and linkage plans in accordance with each client’s age, gender and gender identity, sexual orientation, race, ethnicity, religious beliefs, and culture
  • Determine appropriate type and level of services utilizing the IM-CAT assessment tool
  • Facilitate psychiatric hospitalization when indicated
  • As appropriate, develop amental health assessment or admission note resulting in the identification of the individual's mental health service needs and recommendations for service delivery
  • Work cooperatively with other members of the treatment team including, but not limited to: therapists, security personnel, nurses, crisis workers, psychiatrists and other Case Managers
  • Maintain positive and mutually respectful relationships with members of the Chicago Police Department, other human service providers, entitlement agencies, health care vendors, and the community
  • Assist with outcome measures
  • Always maintain compliance with clinical documentation, timelines and agency policies and procedures including meeting required timeframes for documentation and service notes
  • Report to assigned supervisor and actively seek consultation whenever necessary or requested by supervisor
  • As appropriate, and in accordance with licensure, Case Managers may complete IM+CANS assessments for clients on the waitlist for various programs
  • As appropriate, and in accordance with licensure, Case Managers may engage consumers in skill-based and psychoeducational groups
  • Other duties as assigned by supervisor

Minimum Qualifications:

  • Bachelor’s degree in social work, psychology, or a related field
  • At least 1 year of case management experience preferred
  • Personal vehicle, valid Illinois driver’s license, insurable driving record, and vehicle insurance is required.

Location and Work Conditions:

Although the Case Manager will be based at one of C4’s sites, at least 80% of the services are provided in Chicago communities on the north and west sides and northern suburbs. Work may be stressful due to consumer behaviors, periods of high call volume, or tight deadlines. The Case Manager may be asked to work evenings and occasional holidays.
