JOB SUMMARY FOR ASSISTANT ADMINISTRATOR/SOCIAL SERVICES:

We are currently seeking applicants for the position of Assistant Administrator. This position champions resident advocacy, customer service, and individualized care plans. The Assistant Administrator will assist in planning, organizing, implementing, and evaluating the overall operation of the facility's Social Services Department by current federal, state, and local standards, guidelines, and regulations, and the facility's established policies and procedures.

MAJOR DUTIES AND RESPONSIBILITIES OF THE ASSISTANT ADMINISTRATOR/SOCIAL SERVICES:

The Assistant Administrator will assist the Administrator in the planning, developing, organizing, implementing, evaluating, and directing of social services programs of this facility.

The Assistant Administrator will meet with administration, medical and nursing staff, and other related departments in planning social services.

The Assistant Administrator will participate in the admission process, discharge planning, development, and implementation of care plans and resident assessments.

• Admission areas of focus
• Work with Marketing to facilitate a quality admission process for each resident
• Oversee the completion of admission paperwork, ensuring each resident has a signed contract and advance directive UPON ADMISSION (See Advance Directives Policy for specific procedures)
• Communicate to the staff and IDT the preferences of each new admission
• · Discharge Planning areas of focus
• Identify the discharge goal of each resident upon admission
• Maintain consistent communication with the resident/resident representative as to the status of the resident progress toward the goal
• Support the resident and the IDT to ensure resident participation in the completion of rehabilitation. Encourage residents intending to discharge before completion of rehabilitation.
• Oversee the discharge process including, but not limited to, referrals to home health, home medical equipment, and senior living destinations; establish transportation plans; frequent status communication to IDT
• Monthly report all discharges to the local Ombudsman as per regulation

REQUIREMENTS FOR THE ASSISTANT ADMINISTRATOR/SOCIAL SERVICES:

• Bachelor's degree with the desire to become a Licensed Nursing Home Administrator
• Three (3) years of experience in a Skilled Nursing Facility
• Excellent relationship-building skills
• Ability to work successfully with diverse staff and resident population
• Proven leadership and communication skills
• Must have compassion, tolerance, and understanding for older adults

Pay: $68,000.00 - $72,000.00 per year
