SUMMARY
The Family Service League is seeking a part-time Adult Home Coordinator for our Ombudsman program. The Adult Home Coordinator will provide supervision and mentoring to volunteers and staff assigned to long-term care facilities. The Adult Home Coordinator will be responsible for providing coverage in homes in the absence of the Ombudsman, assisting with certification training, and working with staff on Resident Council Projects. The Adult Home Coordinator will provide support and guidance to Family Councils in the Adult Homes.
RESPONSIBILITIES
- The Adult Home Coordinator will provide supervision and mentoring through telephone contacts and site visits with Ombudsman volunteers assigned to long term care facilities primarily in eastern Suffolk County
- Work effectively with Ombudsman volunteers, residents, families, and staff to resolve complaints and improve quality of care.
- Works with staff to assist and implement Resident Council Support Project.
- Demonstrates knowledge of regulations, codes, and resident's rights.
- The Adult Home Coordinator will interact effectively with service provider agencies, advocacy organizations and regulatory agencies.
- Provide resources and referral information to residents and families and assist them with navigating the long-term care system.
- The Adult Home Coordinator will participate in the certification training of new Ombudsman volunteers.
- Demonstrates cultural competency with communications and complaint resolution.
- Works directly with family members and Ombudsman volunteers to assist with the development of Family Councils within Adult Homes.
- Obtain NYS Ombudsman license/certification.
- All other duties as assigned.
QUALIFICATIONS
Bachelor’s degree in human services, marketing, or related field of study required.
NYS Ombudsman license/certification required.
Minimum of 3 years of experience in working with older adult and disabled individuals required.
Knowledge of community resources or Adult Homes preferred.
Computer proficiency, including Microsoft Office, required.
Excellent verbal and written communication skills, as well as presentation skills, required.
