We are currently seeking a Nursing Health Coordinator for the Adult Day Services program located on the Irondequoit Campus. The primary purpose of the position is to coordinate, supervise, implement and evaluate the health care of participants to uphold the highest quality of life and level of care.

Job Description:
- Maintains health records for all participants and ensures that the quality of nursing care provided is in line with the Standards of Gerontological Nursing and polices of St. Ann's Community
- Performs a pre-admission screening and admission assessment of each participant in a holistic manner with strengths and needs identified
- Monitors and maintains adequate levels of medical equipment and supplies
- Provides or ensures the development of nursing policies and procedures in accordance with St. Ann's Community, NYSDOH Regulatory Agency , NADSA Guidelines, and any Accreditation Agency
- Communicates and coordinates the health care of participants with other disciplines and professionals within St. Ann's and the Rochester community as needed
- Develops and evaluates, with the Interdisciplinary Team (IDT), an individualized plan of care with clear interventions and measurable goals.
Candidate requirements:
- Current/ valid NYS RN/LPN Licensure required
- One-two years' experience in a geriatric setting is required
- Bachelor's degree in Nursing preferred
