Description
The Long-Term Services and Supports Service Area of Human Services seeks an Administrative Assistant to facilitate the timely assignment and transfer of a person to ongoing case management services. The County-to-County Transfer Coordinator position will be responsible for the centralized coordination of work when an individual receiving long-term services and supports moves from another lead agency and Hennepin County becomes the county of financial responsibility.
The programs administered serve seniors, and populations with disabilities including intellectual developmental, brain injury, mental health, complex medical, and physical disabilities. The federal and state programs that serve these populations includes the Alternative Care (AC), Elderly Waiver (EW), Community Access for Disability Inclusion (CADI), Brain Injury (BI), Community Alternatives of Care (CAC), Developmental Disabilities (DD) or Essential Community Supports (ECS).
Location and hours:
This position is remote. Remote workers may be required to come on-site 0 to 12 days a year for work meetings, trainings, or for any other reasons that their supervisor may deem necessary (may include additional days for onboarding and new employee training). Supervisors will provide as much advance notice as practicable. Work hours will be Monday through Friday, with typical hours of 8 a.m. to 4:30 p.m. Based on business needs of this position, hires must live in or relocate to Minnesota.

In this position, you will:
  • Demonstrate commitment to equitable and person-centered practices.
  • Coordinate with external partners, other lead agencies and with internal partners including Front Door, Economic Support, other areas of LTSS (initial assessment, annual assessment, office support).
  • Collaborate with the case management Transfer Team to ensure timely transfer and assignment to ongoing case management.
  • Effectively navigate and utilize systems including Medicaid Management Information System (MMIS), Social Service Information System (SSIS), and Enterprise Communications Framework (ECF) to obtain information about persons served and to document work.
  • Consult with senior social workers on the Contracted Case Management Coordination (CCMC) Team and supervisors on complex situations related to eligibility, assessment, and/or case management needs.
  • Support the development of process and procedures and continuous improvement efforts to ensure continuity of care for persons transferring to Hennepin County.
  • Contact providers to gather information, make referrals and assist persons served in obtaining social services.
  • Work with and maintain an understanding of the assessment and case management processes in LTSS program areas.

Need to have:
  • One of the following:
    • Four or more years of experience in general administrative work related to the management of departmental/organizational operations or in a specialty area appropriate to the position.
    • Bachelor's degree or higher in public administration, business administration, or other field appropriate to the position.

Nice to have:
  • Experience:
    • Strong knowledge of Department of Human Services (DHS) and Hennepin County systems including MMIS, SSIS and ECF.
    • Experience working with home and community-based services (HCBS) waiver programs.
    • Managing documentation in compliance with Hennepin County and DHS requirements.
    • Identifying quality improvement needs and designing and implementing process improvements.
    • Using Microsoft Office Suite, in particular Excel, Word and Outlook, to create reports and spreadsheets.
  • Ability to:
    • Establish and maintain effective working relationships with all levels of staff, customers, and business partners.
    • Manage multiple tasks simultaneously and respond quickly to changing priorities.
    • Understand importance of and protect confidential information.
  • Knowledge of:
    • Using administrative principles and practices relating to the management of department operations.
