Community Action Partnerships of Western Nebraska (CAPWN) is a non-profit community-based health and human services organization that serves low-income, disadvantaged and those unable to meet their needs through other sources. The agency was established in 1965 and provides services to over 9,000 individuals, children, and families in three main areas:
  • Community Health Services: WIC, Commodity Supplemental Food, Foster Grandparents, Migrant Seasonal Head Start, and Child Development Center.
  • Clinical Health Services: Primary Care, Reproductive Health, Diabetes Management, Immunizations, Migrant Health, Ryan White HIV; Behavioral Health including Counseling Services, Medication Management, Medication Assisted Treatment and Substance Use Counseling and Intensive Outpatient Services and Dental Services.
  • Supportive Health Services: Family Stabilization, a Teen Outreach Program, SOAR, Youth Leadership Program, Youth Shelter, and Housing Assistance Programs.

Youth Site Case Manager

Full- Time

Job Title: Youth Site Case Manager

Reports To:Youth Site Manager

Areas of Responsibility:Responsible for assisting for implementation and delivery of supportive and comprehensive case management and advocacy for youth in the program. Assess the needs of the youth, identifying areas of risk and creating a case plan that outlines goals and objectives. Monitor progress on goals and objectives while identifying community resources to assist in creating success in meeting those goals. Interact with youth and provide a clean, safe, consistent, and therapeutic environment; substitute in other youth programs as needed.

Major Duties:

  1. Assist in creating and maintaining a safe, supportive, consistent, and therapeutic environment at the Youth Shelter while acting as a positive role model.
  2. Practice and teach appropriate interventions and logical/natural consequences with each youth including behavioral management techniques.
  3. Provide close supervision of youth to facilitate group and recreational activities and educational development.
  4. Conduct ongoing, individualized case-management to youth in the program.
  5. Maintain a caseload of up to 14 youth.
  6. Assist youth in developing and implementing an individualized program plan to meet personal goals and positive outcomes.
  7. Develop and facilitate support, informational classes, life skills and group activities designed to help clients explore and resolve self-care issues that may have been influencing the client’s progress towards meeting their established goals.
  8. Maintain accurate and confidential case files.
  9. Use Trauma Informed Care and Positive Youth Development principles to implement a positive plan for behavior within the facility.
  10. Communicate goals and progress to Youth Specialists to ensure teamwork for helping the youth recognize success.
  11. Coordinate resources and staff to make sure each youth receives educational, employment, community, and independent living skills training.
  12. Provide coordination of Youth Services programming and facilitate communication between the youth services programs.
  13. Network with and partner with panhandle school systems to provide a seamless continuum of educational care for runaway, homeless, and street youth.
  14. Network and partner with panhandle-wide counseling services to provide crisis prevention, intervention, and post-intervention to panhandle youth.
  15. Record all significant events in resident behavior logs, including goals, observed behavior, medication, phone contacts and other assigned information.
  16. Make appropriate documentation for the intake procedure, service plan, daily log, and discharge plan for each resident.
  17. Maintain open communications through the use of communication log to provide effective consistent and professional levels of care to the youth and families served.
  18. Serve as a mature role model for residents and conduct self in a professional manner by teaching safe respectful boundaries.
  19. Be aware of local, state and national laws, and juvenile codes affecting youth and families.
  20. Attend staff meetings, in-service training and workshops and share information about training received.
  21. Practice appropriate interventions and logical consequences with each youth, which implement a positive plan for the youth’s behavior while at the Shelter.
  22. Monitor case files, data and required documentation for Licensing, RHYMIS, CSBG, ROMA, Volunteer, and Youth Services Contracts/Grants.
  23. Respect the rights and protect the confidentiality of residents and families by adhering to the confidentiality policies of CAPWN.
  24. Accurately follow CAPWN budget guidelines for purchasing.
  25. Participate in community education, public speaking and public relations activities as assigned.
  26. Work with the other staff in program planning, program implementation, and giving program feedback.
  27. Adhere to CAPWN agency policies and office procedures.

  29. Perform other duties as assigned.

Working Conditions:

Generally, on-site work with youth; may require off-site home visits, training, meetings, and community interactions.

Requires a high energy level and the ability to meet deadlines.

Occasional out-of-town travel for training.

Will work evenings and weekend meetings and training.


Physical Requirements:

  • Must be able to move about quickly if necessary.
  • Must be able to lift 30 lbs.
  • Physically able to perform job.
  • Must complete health physical upon entry to program and yearly health screenings.

Minimum Qualifications:

Grade: 16

  1. High school education or equivalent.
  2. Must have attained age 21.
  3. Must have a valid driver’s license, insured vehicle, and good driving record.
  4. Must pass criminal history background check, child abuse/adult abuse and sexual abuse register background checks.
  5. Ability to communicate effectively both orally and in writing.
  6. Minimum one years’ experience working with youth preferred.
  7. Knowledge of age-appropriate child/youth development.
  8. Able to attend meetings and training sessions in or out of the area.
  9. Must have flexible schedule and be able to work primarily evenings, weekends and on-call when needed.

Grade: 17

  1. Must meet minimum requirements above.
  2. AA degree in human service-related field and one year experience; OR a minimum of three years’ experience in a similar position, such as working with runaway and homeless youth, foster care, community outreach worker, family support worker.

Grade: 18

  1. Must meet minimum requirements above.
  2. AA degree in human services related field plus three years’ experience, OR BA degree in human services, psychology or social work and one years’ experience, or specialized training.
