Job Summary:
The Wraparound Care Coordinator assists with facilitation of the Discovery Process (Strengths, Needs, Cultural, Vision, and Discovery) during home or office visits with child and family. Duties may include: invite wraparound team members, set up meetings, writes the wraparound plan of care for families, assists with crisis plan and assists with case management for wraparound process. Wraparound coordinator will also assist with Transportation for therapy groups. Will also assist as a Horse Handler with Equine therapy groups and camps as needed.
Duties & Responsibilities:
Employee will be given orientation to Wraparound Care and Manual to read.
Will assist with the following:
- Provides Home, Office or Ranch Visits to facilitate Discovery Process with children and families.
- Invites wraparound team members & sets up wraparound meetings.
- Writes wraparound plan of care for child and family during/after meeting.
- Provides referrals and case management for wraparound process.
- Arranges for Lakota language translation services as needed for families.
- Will assist with Horse Handling for Equine Therapy group sessions as needed.
- Coordinates Flex Fund requests and starts approval process for eligible relatives.
- Completes all required documentation for Team File for care coordination.
- Attends monthly TGKP Team Case review Meeting.
- Attends Tiwahe Glu Kini Pi Staff Meetings.
- Assists with transportation for children to groups and families as needed when insured.
- Attends staff and partner trainings.
- Assists with Tiwahe Glu Kini Pi Community Wellness activities.
- Other duties as agreed upon with supervisor.
Qualifications & Experience:
Must have Horse Handler skills.
- Must possess some knowledge of Lakota culture, philosophy, history, language & spirituality.
- Good communication and networking skills with youth and families.
- Good organizational & technology skills.
- Must be Alcohol & Drug Free.
- Must have a valid Driver license if able to transport.
- Must be able to pass a background check.
