POSITION TITLE: WellnessCase Manager STATUS: Exempt, FT or PT GRADE: 4

DEPT: SVSS PROGRAM: Clinical Program SALARY: $45K/yr

REPORTS TO: Associate Director Case Management SUPERVISES: N/A


SUMMARY: The Case Manager assists youth, adolescents and adults experiencing a range of mental health challenges including sexual violence in assessing basic needs and assists them in acquiring resources that promote health and well-being. The Case Manager actively builds and fosters effective working relationships with other SVSS staff members, community partners, and promotes synergy across YWCA programs.

COLLABORATES WITH: The Case Manager collaborates with the Associate Director Case Management, other SVSS staff members; members; Volunteers; YWCA staff members and community partners.


ESSENTIAL responsibilities:

Direct Service: Provide direct services to clients seeking mental health services to support appropriate use of all needed health care services. Collaborate with clients to develop and implement goal-oriented and integrated plans, based on overall needs and interests. Assess clients basic needs for emergency resources; physical and emotional health; and other wellness components, such as social, occupational, and intellectual. Assist clients in acquiring basic elements of self-sufficiency: IDs, medical coverage, birth certificates, U.S. Social Security cards, Employment Authorization Documents, etc. Assist clients with accessing private and public resources and provide enrollment assistance as needed. Maintain an active caseload of designated clients. (65%)

Member Data Management: Maintain comprehensive member records as required. Maintain all data entry and statistical records for multiple funding sources regarding case management services performed. Maintain member confidentiality as required by law and funding sources. (5%)

Professional Development & Case Management Knowledge: Collaborate with other Case Managers and participate in team meetings. Participate in regular reflective supervision meetings as required. Complete twelve hours per year of continuing education in providing case management services, with at least six of those hours addressing the specific member populations served. Complete other continuing education and training as set by agencies/funders. (10%)

Community Engagement: Create, cultivate, and deepen strategic partnerships with public and private organizations and groups to establish valuable and responsive referral connections. Participate and provide leadership in appropriate local mental health and human services coalitions. Consult with outside community members and groups to help design and implement new services, or target underserved populations, as needed. Assist with planning and implementing special programs e.g. Mental Health Awareness Month, "Take Back the Night March." (20%)


NON-ESSENTIAL RESPONSIBILITIES: May be asked to perform public speaking responsibilities specific to programming and public policy issues and carry representative responsibilities to other groups, agencies, and organizations. May recruit, train and evaluate and/or supervise program volunteers.


OTHER DUTIES AS ASSIGNED: Other duties outside of the responsibilities of the position may be assigned, whether for a project, special needs task or other assignment.


QUALIFICATIONS: A minimum of one year of experience in case management is required. Previous experience in working with trauma populations, specifically with survivors of community violence preferred. Previous training in sexual assault or related issues is highly preferred. The ability to work collaboratively or independently, as situations require, is necessary. The position requires availability to work evenings and some weekend days (Saturday)*. Scheduling purposes require a basic skill level in MS Outlook; specific program data entry needs require skills in MS Office, specifically in Excel. Knowledge of databases such as Infonet and ETO is advantageous.


Skills in creativity, innovation and ease in engaging members are highly preferred. The ability to make sound decisions in emergency situations is necessary. Highly effective interpersonal and communication skills with victims/families are required. Social perception and cultural awareness are required for this role. The role requires a strong service orientation and complex problem solving skills. The Case Manager must be effective regarding use of time and able to prioritize work assignments.

Professional Training Required: Complete the ICASA forty-hour sexual assault crisis intervention training within thirty days of employment start date (if not completed within the prior two years). Complete other continuing education and training as set by ICASA or other agencies/funders.

Note: A bilingual Case Manager must have cultural competency and conversational and written fluency in the language of the population that is served.


EDUCATION REQUIRED: A Bachelors degree BA, BSW, etc. from an accredited college or university in Psychology, Social Work, or related Human Services field.

WORK ENVIRONMENT: The position requires working in an office where survivors of violent crime and assault and their family members, are present. As such, the Case Manager must be able to professionally handle unexpected crises. The essential functions of the position require operation of computers and other office equipment and movement between offices. The noise level is usually moderate. Movement requirements include walking, stooping or kneeling. There may be a need to reach or to lift objects, such as packages or supplies. Communication needs include seeing, hearing and speaking. While in the office, there will be periods of sitting, holding a telephone and using computers. Travel to other locations in the city or other regions of Illinois may be required. Reasonable accommodations will be made for an incumbent to meet the essential responsibilities of the position.

WORK SCHEDULE: Monday Friday, this position requires at least one evening per week and some Saturdays to meet client and program needs; schedule may vary depending on the needs of the agency.
