Under the direction of the VNA Manager and within established organizational and departmental policies and procedures, responsible for providing services to address the social and/or emotional factors that impact the patient's ability to achieve the goals in the Plan of Care. Services are provided under the direction of a physician and in conjunction with other health care team members.
- Master's degree from a school of social work accredited by the Council on Social Work Education required. Current license as a LCSW required.
- Minimum of one (1) year experience in a health care setting required, home care experience preferred.
- Demonstrated strong verbal and written communication skills and good interpersonal skills required.
- Demonstrated knowledge of the principles and practices of social work and of resources available in the community required.
- Current driver's license, good driving record and access to or own reliable transportation required.
