Title: Case Mgr/Discharge Planner/Utilization Review

Location: Meeker, CO 81641 ( Only Travelers are accepted.)

*****RADIUS RULE: Traveler’s perm address must be at least 65 miles away from our facility. Local travelers will not be considered.

Shift Information: 8am - 5pm Monday - Friday.

Certifications/Requirements:

• Covid Card
• BLS Card
• RN License in CO.
• CCM

ABOUT: 16 bed Critical Access Hospital with 1 Case Manager located in Meeker, CO

Patient Ratio: less than 1: 15 1- Average daily census is 4.7

COVID Vaccinations: required per State mandate

Pay: $70.00 - $75.00 per hour

• Utilization review: 3 years (Required)

Ability to Relocate:
