Job Title:RN Home Health Case Manager
Location: Denver Metro Area, Colorado
Salary: Starting at $42 per visit
Schedule: Flexible schedule, Monday through Friday
Job Description:
We are currently seeking a Registered Nurse (RN) to join our team as a Home Health Case Manager in the Denver Metro Area. As a Case Manager, you will be responsible for providing compassionate and comprehensive Home Healthcare services to our patients.
Responsibilities:
- Conduct initial patient assessments to develop personalized care plans.
- Provide skilled nursing care to patients in their homes.
- Monitor and evaluate patient progress, adjusting care plans as necessary.
- Coordinate with other healthcare professionals and community resources to ensure continuity of care.
- Educate patients and their families about their conditions and treatment plans.
- Document all patient care activities accurately and promptly.
Qualifications:
- Current Colorado RN license.
- Minimum of 1 year of experience in Home Health or a related field.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Valid driver's license and reliable transportation.
