As an Unlicensed Intake Coordinator, you will play a crucial role in our mission by coordinating intake processes, facilitating seamless transitions for new clients, and ensuring the highest standards of care are maintained.

Key Responsibilities:

• Conduct initial intake assessments over the phone or in person to determine clients' needs and eligibility for services.
• Coordinate with healthcare professionals, clients, and their families to develop personalized care plans.
• Collaborate with internal teams to ensure timely and accurate documentation of client information.
• Serve as a liaison between clients, caregivers, and healthcare providers to address any concerns or inquiries.
• Provide exceptional customer service by addressing client inquiries and concerns with empathy and professionalism.
• Assist in scheduling and coordinating client visits and services as needed.
• Telemarketing the home health services.

Qualifications:

• High school diploma or equivalent required
• Previous experience in healthcare coordination, customer service, or related field is a plus.
• Excellent communication and interpersonal skills.
• Strong organizational abilities with attention to detail.
• Ability to work collaboratively in a fast-paced environment.

Job Types: Part-time, Contract

Pay: $16.00 - $17.00 per hour

Expected hours: 20 per week
