Job Title: Transportation Dispatcher
Job Summary: Under the supervision of the Transportation Manager, the Transportation Dispatcher is responsible for planning, organizing, and implementing the transportation services for POSP participants. This position requires constant interaction with the Interdisciplinary Team (IDT) Center Managers, participants, caregivers, and community agencies. This position is responsible for serving as the Transportation representative of one or more IDT Teams.
Education: High School Diploma or Equivalent.
Experience: Must be 18 years old and have a minimum of l year of experience working with the frail elderly population.
Number and Type of Employees Supervised (optional): Participant Services Representative
Licensure, Registry or Certification Required: Valid NC Driver’s License with safe driving record with a minimum of no major moving violations
Specifications: Must be able to work effectively in a team environment and operate handicap accessible vehicles. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by PACE of the Southern Piedmont and approved by CMS before working independently and yearly thereafter.
Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Location: Pace of the Southern Piedmont, Charlotte, NC
Key Responsibilities:
• Coordinates and plans all transportation trips for participants for Senior TLC daily attendance, specialty appointments, discharge from SNF (Skilled Nursing Facility) & hospital, and daily dialysis trips including weekends and holidays.
• Works independently and makes scheduling decisions for the organization based on resource allocation and participant needs. Determines the safest and most efficient pick-up and delivery routes. Receives feedback from Drivers to integrate into transportation schedules.
• Daily receives information from IDT meetings and care planning as scheduled. Effectively communicates with participants, families, staff, and other agencies and departments.
• Provide training and support to a team of Drivers in collaboration with the Transportation Manager to ensure competence in driving a variety of vehicles that are included in the organization's transportation fleet.
• In collaboration with the Transportation Manager ensure that the team of Drivers meets competencies for driving a specially equipped van to deliver physically limited persons to varied destinations. Monitors and ensures that all Drivers are trained in the proper operation of wheelchair lifts and tie-down equipment and that they are properly operated daily.
• Ensure that all participants are delivered to appointments promptly.
• Attends staff meetings. Maybe a member of the IDT teams and communicate any changes in participant conditions to the Center Manager. Responds to emergency situations appropriately.
• Maintains a safe driving record as evidenced by retaining a North Carolina driver's license. A valid driver's license will be verified annually.
• Must be able to meet all physical requirements of the position including standing, bending, reaching, pulling, pushing, squatting, and walking for intermittent periods of time.
• Participates in participant care planning including the implementation of SMART goals/interventions for the participants' care plans and enters all care plan information in a timely manner as per organizational protocols.
• Documents assessments, incidents, and other participant information appropriately in all forms of documentation resources including the participants' electronic medical record.
• Assist with special projects assigned by the Transportation Manager.
• Other duties as assigned by supervisor.
