Concordia Hospice of Washington is seeking a Team Assistant for a full-time position which will report in our Hospice IPU, located in Washington, PA.
POSITION SUMMARY
The Team Assistant assists and performs various duties to support daily operations that may include coordinating, monitoring and evaluating options and services for patients and clinical staff.
QUALIFICATIONS
- Knowledge of Hospice regulations
- Unit clerk experience at hospital or facility preferred
- High school education required
- At least two [2] year experience in a health care environment
- Computer skills: Word and Excel Spreadsheets
- Excellent organizational skills with attention to detail
- Outstanding communication and interpersonal skills both written and verbal
- Ability to prioritize and organize daily work load
- Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
- Reviews all incoming paperwork for documentation, compliance and completeness
- Enters patient demographic and clinical data into current software system
- Receives referrals from intake and disperses them to the appropriate individual
- Verifies daily visit records with schedules and visit reports for statistical and payroll purposes
- Verifies visits and authorizations for disciplines via the computer
- Assists in preparing management reports of patients information as requested
- Alphabetizes paperwork and files appropriately
- Tracks all insurance authorization visits and enters the information into the system per agency protocol
- Assists in gathering relevant statistical data
- Assists in maintaining schedules for all staff
- Generates and distributes reports from various software systems for management
- Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards, and Concordia Hospice of Washington policies and procedures; maintains confidentiality of patient and employee records and information; has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to the Executive Director and/or Compliance Manager
