At Yellowstone Boys and Girls Ranch, we count on our Targeted Case Managers to support a trauma-informed environment while primarily providing Targeted Case management services to youth and families with social, emotional, and behavioral problems. The four core areas of Targeted Case management are assessment, development of service plans, referring activities, and monitoring of services.

A Day in the Life of Targeted Case Manager:
- Advocate, support, and guide youth and families in obtaining the needed services including, but not limited to, advocacy, coordination of the various services, education regarding community resources, and referral of a youth/family to available services within the community.
- Maintain contact with other service providers to ensure the unique needs of the youth and family are best met.
- Research the various services offered within the community to identify services that would best meet the unique and individual needs of youth and families.
- Work with the youth, the youth's family, and other professionals involved to develop a treatment/service plan that addresses the specific needs of the youth including, facilitating, and leading the treatment team meetings.
- Participate and engage in monthly supervision/coaching meetings with the supervisor.
Qualifications
What You Need to Qualify:
- A Bachelor's degree in a mental health-related field, human services, social work, psychology, or related field of study is required.
- An equivalent combination of education and experience may be considered in lieu of a degree.
- 21 years of age or older
- Driver's License to operate a passenger vehicle.
- Display regular, predictable, and reliable attendance at work
- Willingness and ability to obtain certification in First Aid/CPR and Violence Intervention Prevention.
- Successfully complete Child and Adolescent Service Intensity Instrument (CASII) training within 30 days of hire.
- Knowledge of the specific treatment/mental health needs of youth with emotional and psychiatric problems and their families
- Excellent written and verbal communication skills
- Knowledge of local community resources for youth and families.
- Good computer/technology skills. Able to quickly learn and use YBGR selected software solutions.
- Criminal / Child Abuse Clearance: Must pass criminal record and child abuse registry background check as required by the State of Montana.
Preferred Qualifications
- 2 (two) years of professional experience in case or treatment management is preferred.
- 2 (two) years of professional experience working with youth with serious emotional disturbances is preferred.
