TARGETED CASE MANAGER
Job Description
Education: Bachelor's degree from accredited university or college with major in: counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education or a related human services field in compliance with Medicaid requirements.
Experience: One-year target case management experience providing case management service.
Responsibilities: Conducts the initial assessment and develops the recipient service plan according to Florida Medicaid guidelines. Advocates for the acquisition of services and resources necessary to implement the service plan by representing or defending recipients through direct intervention. Prepares written plans concerning social level skills and education.
Candidate must be certified by The Florida Certification Board as Target Case Manager or willing to get certification 90 days from beginning date.
Job Types: Full-time, Part-time, Contract
Pay: $22.00 - $24.00 per hour
Expected hours: 25.00 – 40.00 per week
