Job Description
We seek a self-motivated and compassionate individual with experience working with adults
with intellectual and developmental disabilities.
Qualifications
- One year of proven experience working with adults with developmental disabilities.
- Candidates must be able to take the initiative, work independently, make executive decisions when necessary, and meet deadlines.
- Excellent written and verbal communication, reliable car, flexibility to attend meetings when needed during the day.
- This hybrid position requires field visits; candidates must be flexible to travel withinthese counties.
- The state of New Jersey requires Covid vaccination for this position.
- Must pass criminal background and CARI check.
- A bachelors degree in psychology, sociology, social work, or related field.
- Assessing service needs, developing the New Jersey Individualized Service Plan (NJISP)for access to services, and liaison with clients, families, programs, and service providers.
- To establish achievable goals, outcomes, and service eligibility.
- Development of a Person-Centered Planning Tool (PCPT) is a mandatory discovery toolused to guide the person-centered planning process and assist in developing the NJISP.
- Ongoing monthly monitoring of clients.
- Coordinate and facilitate team meetings off-site at either Residential and/or DayPrograms to identify service needs.
Pay is $1,500 - $4,500 Monthly, commensurate with experience and caseload.
Must reside in the State of New Jersey
