SUMMARY: Open Door Community Health Centers (ODCHC) is committed to providing a comprehensive array of services to our surrounding communities. Our Substance Use Disorder (SUD) Counselor work one-on-one and in groups with patients to help them overcome Substance Use dependencies. The SUD Counselor facilitates support groups, provides one-on-one counseling, assists patients integrate into the addiction support services, including the Medication Assisted Treatment (MAT) program and primary medical care services of ODCHC, and works alongside patients and families to create a care plan specific to their needs. This position requires a hardworking individual who is patient, caring and community-minded. Experience and familiarity with community resources is preferred. Must have obtained certification as a Substance Use Disorder (SUD) Counselor from one of California's Drug and Alcohol Counselor certification programs.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
- Interview clients, review records, and confer with other professionals in order to evaluate individuals' mental and physical condition, and to determine their suitability for participation in specific programs and treatment plans.
- Perform intake interviews, confer with medical provider on assessments, information, referrals, and counseling for patients with substance abuse issues to support individual rehabilitation. Facilitate support groups; assist with the integration of the MAT Program and connection to local Needle Exchange Programs.
- Work with selected patients, upon referral from the providers, to develop, implement, and monitor an effective treatment plan. Provide individual, group and family counseling focused on overcoming Substance Use dependencies.
- Review and evaluate clients' progress in relation to goals described in treatment and care plans.
- Plan and implement follow-up and aftercare programs for patients.
- Provide patient, family members and/or care providers with information about addiction issues and about available services and programs, making appropriate referrals when necessary.
- Monitor barriers and access to care issues, make recommendations to management on areas for future attention.
- Maintain current information on community resources, programs, and services.
- Maintain appropriate records and prepares reports. Maintain documentation for various enabling programs.
- Participate in quality improvement and risk management activities.
- Discuss systemic challenges and difficult situations with supervisor on a regular basis.
- Contribute positively to the efficiency, accessibility, productivity, quality, safety, compassion and professionalism of the work setting.
- Participate and/or facilitate team meetings at their site.
- Other duties and responsibilities as identified and assigned by supervisor.
- Strong interpersonal skills and the ability to establish constructive professional working relationships among a diverse workforce and patient population.
- Knowledge of methods and techniques of individual, group and family counseling; and psychological, physiological and sociological aspects of substance abuse.
- Ability to work as a member of a team and independently as needed.
- Ability to handle difficult or confrontational situations in a calm, consistent, and equitable manner.
- Ability to write routine reports and correspondence.
- Ability to effectively present information and respond to questions from patients, practitioners, support staff, and the general public.
- Ability to maintain confidentiality and objectivity.
- Strong computer skills with specific aptitude in Microsoft Office Suite and/or Electronic Medical Records (EMR).
- Ability to effectively represent ODCHC's interests in the community and maintain effective working relationships among coworkers, public, private, and professional groups.
- Experience and familiarity with community resources.
- Adheres to ODCHC's policies and procedures.
- High school diploma or GED. Higher education or class work in Alcohol and Other Drugs, Substance Use Disorders, or Addiction Studies preferred.
- Two years' experience providing drug and alcohol counseling and education services preferred.
- Maintain certification as an SUD Counselor from one of California's Drug and Alcohol Counselor certification programs. Qualifying certifications include Certified Alcohol and Drug Counselor (CADC); International Certified Alcohol and Drug Counselor (ICADC); Licensed Advanced Alcohol and Drug Counselor; Substance Use Disorder Certified Counselor.
PHYSICAL REQUIREMENTS: This is largely an office-based position. The physical requirements described are representative of those needed to successfully perform the essential duties of the position.
