Job Description
Status: Either Full-time (25-40 hours/week) or Part-time (20-26 hours/week)
Summary
Provides quality AOD and/or Mental Health treatment services to clients seeking support with Substance Abuse and/or mental health. Provides comprehensive diagnostic assessments, interventions, education, referrals, and consultations for clients in an outpatient setting. Achieves productivity objective of at least 65% per week of hours worked.
Responsibilities:
• Provides effective individual and/or group interventions and education to support behavioral change
• Cultivates education, administration and self-management skills for clients
• Promotes smooth interface among Clinical, Medical and other specialized healthcare providers
• Coordinates client treatment plans and serves as point of contact for client and participating providers (Medical staff, Case management Staff, Physicians etc.).
• Monitors participation and progress of clients assigned to their caseload and provides and intervenes appropriately when participation or progress is inconsistent with individual treatment plan
• Provides onsite availability for crisis triage, intervention and case management services
• Documents the clinical behavioral health aspect of client interventions and tracks client referrals to specialty external behavioral healthcare
• Attends staff meetings as required and participates in weekly clinical supervision
• Formulates plan of care (Individual Service Plan or ISP) for assigned clients and completes 90-day reviews of the ISP for each assigned client
• Documents client assessment and intervention actions in Electronic Medical Records (EMR) system, using established forms/formats and documentation practices, in a timely manner
• Provides SAMHSA curriculum to individuals and groups about AOD and/or Mental Health treatment and recovery
• Communicates ideas and skills that can change client knowledge, attitudes, values, behaviors
• Establishes appropriate relationship with clients and client family members that serve to enhance the clients’ capacity for proper self-care and coping while consistently maintaining professional distance and respecting professional boundaries
• Participates as a member of a multidisciplinary team to optimize client outcomes
• Maintains required licensure
• Maintains professional competence through continuous learning, utilizing various methods of self-education and instruction
• Adheres to all applicable standards and policies of privacy, security, safety, confidentiality and compliance, including applicable laws and regulations (Examples: HIPPA, OSHA)
• Consistently achieves personal productivity objective of at least 65% (productivity is calculated by dividing number of billable hours by the number of hours worked each week); seeks assistance from Supervisor as needed to ensure adequate knowledge and skill needed to achieve this objective.

Qualifications:
Minimum (dependent on role assignment):
• License: Current CDCA
• Education: AS degree in mental health or related field; significant additional experience may be considered in lieu of degree
• Experience: At least 1 year applicable experience
• Demonstrated documentation skills; demonstrated proficiency in MS Word and MS Outlook email systems
• Demonstrated individual counseling or group facilitation skills
• Demonstrated capability in utilization of EMR systems
• Must pass background verification and drug screen
Preferred:
• License: Current LCDC/LICDC; LSW/LISW; LPC/LPCC; or similar
• Education: Bachelor’s or master’s degree in social work, criminal justice, psychology, social work, counseling
• Experience: 3 or years applicable experience
• Demonstrated documentation skills
• Demonstrated individual counseling and group facilitation skills
• Demonstrated capability in utilization of Credible EMR system
Physical Requirements:

Work Environment:
Work may be performed in a variety of environments and settings, including climate controlled offices, sober living residences provided by third party, jails or other incarceration facilities and in the outdoors. Incumbent may be exposed to temperature extremes, sub-standard sanitary and living conditions. Incumbent may be exposed to unstable client behavior.

Performance Expectations:
Among other things, incumbents will be evaluated on their ability to model the job Competencies noted above, the quality of the service they provide, and productivity. The productivity standard for this position is that 70 % of all hours worked are billable.
