POSITION DESCRIPTION
POSITION TITLE
Student Service Nurse (RN/LPN)
QUALIFICATIONS
  1. Graduate from an accredited school of nursing.
  2. Possesses a current license to practice as a professional registered nurse or license practical nurse in the State of Arizona.
  3. Recent experience in the field of public health nursing and/or school nursing preferable.
  4. Valid Arizona State Fingerprint Clearance Card.
  5. Valid Arizona State Driver's License.
  6. Capable of performing independently with a high degree of competence and professionalism responsibilities pertaining to the job description.
  7. Bilingual English/Navajo preferred.

JOB GOALS
To provide health services to general education children and to enable a child with a disability to receive Free Appropriate Public Education (FAPE) and serving as a team member in providing preventive services, early identification of problems, interventions, and referrals to foster health and educational success. If there is an educationally relevant medical finding, or health condition, that is impacting the student in the educational setting, the nurse, will make recommendations to Multidisciplinary Evaluation Teams (MET) so that health services can be provided accordingly.
PERFORMANCE RESPONSIBILITIES
  1. Acts as the liaison between the student primary care provider(s) in the school setting to meet the specialized health care needs of the student-client during the school day.
  1. Supports early intervening services through the Child Find process, such as Multi-tiered System of Supports (MTSS), Response to Intervention (RTI), and universal design for learning, through direct and consultative services.
  1. Assists MET in observing and describing concerns regarding students with disabilities with significant health concerns.
  1. Conducts thorough screening and assessments; interprets results to MET and writes appropriate recommendation reports.
  1. Develops, monitors and reports on school-based health services for students with Individualized Education Plans (IEP) that support specially designed instruction.
  1. Provides direct service and individualized services to meet the health needs of the students.
  1. Consults with school staff, parents and agencies regarding student health needs, progress, and instructional accommodations and/or modifications for the daily activities of students with health needs.
  1. Addresses the physical, mental, emotional and social health needs of students and supports their achievement in the learning process.
  1. Maintains documentation, including relevant data needed to recommend and monitor student's response to health services, session notes and screening summaries.
  1. Keeps up-to-date in knowledge of health requirements and services, including, parental rights and responsibilities, and special education requirements and procedures.
  1. Assists with eliminating health related barriers to learning.
  1. Ensures that school personnel are appropriately and adequately prepared and trained to support student's health plans; educate patient and family on best healthcare practices and how to advocate for the patient.
  1. Possesses knowledge of and/or is familiar with Americans with Disabilities Act.
  1. Collaborates with family, school staff and other health clinicians to create individualized plans for work or school environments.
  1. Creates recommendations for medical plans through analysis of patient's health information.
  1. Ensures that mandated health screening programs and reports are carried out as required by the Department of Education and Department of Health Services.
  1. Participates in School and District Safety Committee(s), Emergency Response Teams and Crisis Management Teams, and assists in developing emergency protocol and procedures for the District and to ensure students with disabilities are addressed in school safety protocols.
  1. Provides in-service education and serves as a consultant to teachers and school staff members on topics concerning health services based on student's IEP.
  1. Develops Individual Health Care Plans (IHCP) and Individual Emergency Plans (IEP) for certain chronically ill students to be followed by staff responsible for the child's care, using physician input when necessary.
  1. Confers with teachers, emphasizing confidentiality, on matters related to the welfare of certain medically fragile students in the classroom.
  1. Assists school health technicians in coordinating with Kayenta Indian Health Service clinics for vision, dental, and dental sealants schedules on an annual schedule for students with disabilities, specifically with students who have chronic medical needs.
  1. Assists and recommends to the administration the formulation of school health programs, activities, and health policies and procedures for students with disabilities with chronic health needs.
  1. Participates in professional growth activities.
  1. Attends local or State meetings related to school health issues for students with disabilities and relays information back to the District.
  1. Maintains a high level of ethical behavior and confidentiality.
  1. Performs other duties as assigned by supervisor.

TERMS OF EMPLOYMENT
When first hired, employee will serve a 90-day probationary period. Thereafter, a full-time contract for 10 months (204 days) a year will be awarded as an Exempt position and placement on the Initial Student Service Nurse Salary Schedule. The Governing Board will establish salary and work year.
