Form of workResponsibilities Jefferson Speciality Pharmacy Patient Care Coordinator- Full-time, Days. This is an on-site position, Location: 3500 Horizon Drive, KOP Handle multi-channel interactions in a centralized contact center environment from patients and families to schedule, receive, handle and triage incoming phone calls to the Specialty Pharmacy Call Center. Handles other miscellaneous tasks as assigned, including investigating expired prescriptions and encounters as necessary.

Interact with a diverse patient base to ascertain the reason for the call and assist the caller with their questions, concerns or problems with a focus on first call resolution. Demonstrate commitment to service and follow-through.
Follow departmental protocols to ensure patient safety and accurate data entry. Schedules patient medication deliveries, reviews chart notes, relays information about medications to callers, and escalates calls as needed to pharmacists or Specialty Pharmacy Representatives. Utilize Electronic Medical Record (EMR) to complete delivery scheduling, verify demographic information, review chart notes, answer inquiries and resolve user issues in accordance with established protocols to ensure patient safety, compliance and departmental policies and procedures.
Coordinate referrals and insurance requirements by obtaining and/or confirming patient information to ensure the patient receives maximum benefits for services. Assure regulatory and compliance requirements following local pay or coverage determinations.
