What you will do:

• Responsible for consultation and directions o Social Servies and Swing Bed in the Hospital.
• Provides orientation and education to Medical and Nursing Staff.
• Develops, Organizes, and implements programs, policies, and procedures.
• Conducts case work services and counseling.
• Accountale for Sing Bed Program

Responsibilities:

• Assures that social service/discharge planning activities are integrated and coordinated within the facility by effective communication at all levels. Conducts patient care conferences and aid in developing a care plan that is developed on all department assessments.

   

• Develops specific, measurable, realistic plans and objectives which enhance social service delivery.

   

• Demonstrates the ability to perform a psychosocial assessment on all patients and reassessments, including Obstetrics and Pediatric populations.

   

• Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.

   

• Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate.

   

• Demonstrates knowledge of clinical social work and discharge planning.

   

• Treats patients and their families with respect and dignity.  Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Interviews Swing Bed Patients and family for admission process and explain the program.

   

• Manages and operates equipment safely and correctly.

   

• Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.

   

• Communicates appropriately and clearly to physicians, staff, Director of Nursing and senior administrative team and acts as a liaison among Swing Bed patient and family, nursing staff, physician, and administrator.

   

• Coordinates and directs to ensure patients’ needs are met and facility policy is followed.

   

• Demonstrates an ability to be flexible, organized and function under stressful situations.

   

• Maintains a good working relationship both within the department and with other departments.

 

 

 

• Consults other departments as appropriate to collaborate in patient care and performance improvement activities.

 

 

 

• Maintains performance improvement activities for the department.

 

 

 

• Duties as Assigned.

 

 

 

• Meets current documentation standards and policies.

 

 

 

• Provides education to staff on performance improvement. Plans and organizes orientation and in-service training for staff members; participates in guidance and educational programs.

 

 

 

 

Qualifications

What you will need:

• Current Clinical Social Work licensure in State of North Dakota
• BSW required, MSW preferred.
• 3 or more years of previous experience in all aspects of social services and discharge planning.
