Social Worker/Swing Bed Coordinator

What you will do:
- Responsible for consultation and directions o Social Servies and Swing Bed in the Hospital.
- Provides orientation and education to Medical and Nursing Staff.
- Develops, Organizes, and implements programs, policies, and procedures.
- Conducts case work services and counseling.

Responsibilities:
- Assures that social service/discharge planning activities are integrated and coordinated within the facility by effective communication at all levels. Conducts patient care conferences and aid in developing a care plan that is developed on all department assessments.
- Develops specific, measurable, realistic plans and objectives which enhance social service delivery.
- Demonstrates the ability to perform a psychosocial assessment on all patients and reassessments, including Obstetrics and Pediatric populations.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning; family is included in teaching, as appropriate.
- Demonstrates knowledge of clinical social work and discharge planning.
- Treats patients and their families with respect and dignity. Identifies and addresses psychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families. Interviews Swing Bed Patients and family for admission process and explain the program.
- Manages and operates equipment safely and correctly.
- Interacts professionally with patient/family and involves patient/family in the formation of the plan of care.
- Communicates appropriately and clearly to physicians, staff, Director of Nursing and senior administrative team and acts as a liaison among Swing Bed patient and family, nursing staff, physician, and administrator.
- Coordinates and directs to ensure patients’ needs are met and facility policy is followed.
- Demonstrates an ability to be flexible, organized and function under stressful situations.
- Maintains a good working relationship both within the department and with other departments.
- Consults other departments as appropriate to collaborate in patient care and performance improvement activities.
- Maintains performance improvement activities for the department.
- Duties as Assigned.
- Meets current documentation standards and policies.
- Provides education to staff on performance improvement. Plans and organizes orientation and in-service training for staff members; participates in guidance and educational programs.
Qualifications
What you will need:
- Current Clinical Social Work licensure in State of North Dakota
- BSW required, MSW preferred.
- 3 or more years of previous experience in all aspects of social services and discharge planning.
