Job Title:Licensed Independent Social Worker (LISW)
Job Type: Full-Time (Part-Time Considered)
We are seeking a Licensed Independent Social Worker (LISW) to join our team. The LISW will play a pivotal role in providing holistic care to our patients, with a focus on pain management, addiction medicine, and behavioral health. This position offers both full-time and part-time options, with the opportunity for some remote work to provide telehealth services.
Qualifications:
- Current licensure as a Licensed Independent Social Worker (LISW) in South Carolina
- Master's degree in Social Work (MSW) from an accredited institution
- Strong knowledge and experience in pain management, addiction treatment, and behavioral health
- Excellent interpersonal and communication skills
- Ability to work independently and as part of a multidisciplinary team
- Empathy and sensitivity when working with patients facing complex challenges
Responsibilities:
Assessment and Treatment Planning:
- Conduct comprehensive assessments of patients' physical, emotional, and social needs.
- Develop individualized treatment plans that address pain management, addiction medicine, and behavioral health concerns.
Therapeutic Interventions:
- Provide evidence-based therapeutic interventions to support patients in managing pain, overcoming addiction, and improving their mental health.
- Conduct individual and group therapy sessions.
Patient Education:
- Educate patients and their families on pain management strategies, addiction recovery tools, and behavioral health resources.
- Promote self-care and healthy coping mechanisms.
Case Management:
- Collaborate with healthcare providers and external agencies to coordinate care and ensure a holistic approach to treatment.
- Monitor and update treatment plans as needed.
Documentation:
- Maintain accurate and up-to-date patient records, treatment plans, and progress notes.
- Comply with all legal and ethical standards for documentation.
