Baptist Health Care Center a 140-bed senior care campus is hiring a Part-Time Social Worker / Admissions Designee to join our team. In this role, you will advocate for our residents in long-term care and family members to ensure they receive high-quality care and services. This role will also assist in reviewing and processing admissions.

What does a Social Worker/ Admissions Designee do?:
Weekly Job Duties
- Provide support to 140 residents and their families in medical, emotional, physical, or psychological concerns.
- Assist with MDS interviews including BIMS and PHQ 9.
- Conduct tours and assist residents with adjustment to their new surroundings.
PRN Job Duties:
- Fill in for vacation days of Social Workers or Admissions Coordinator
- Provide services by completing MDS’s, CAA’s, and Care conferences with families.
- Works in conjunction with another full time Social Worker as well as an admissions coordinator.
- Assist the team in coordinating social services for the facility, writing individualized plans of care for the residents, medical equipment, counseling or other follow-up treatment plans.
- Commitment to compassion, excellence and quality care.
- Act as liaison between families, staff, and various agencies.
- Act as an advocate ensuring basic human and resident rights.
- Participate in admissions, provide appropriate referrals and discharge planning of residents.
- Provides resources and referrals to staff, residents, and family members.
- Helps to develop individualized care plans for residents with behavioral symptoms or psychiatric illness.
What does a Social Worker/ Admissions Designee require?:
- Prefer a Licensed Social Worker, LBSW, but those without a Social Worker degree would be considered a Social Worker Designee, which would also be considered
- Experience working in LTC preferred.
- Must have knowledge of state and federal guidelines related to long-term care and resident rights.
- Excellent customer service/people skills.
- Team player and self-starter.
- Successful candidates will be mission-minded with excellent communication skills.
- High School Education or GED.
