Responsibilities: - Provide counseling and support to individuals, families, and groups - Conduct assessments to determine clients' needs and develop treatment plans - Collaborate with other healthcare professionals to coordinate care for clients - Assist clients in accessing community resources and services - Advocate for clients' rights and ensure their well-being - Maintain accurate and up-to-date client records and documentation - Participate in case conferences and team meetings Qualifications: - Bachelor's or Master's degree in Social Work or related field - State licensure or certification as a Social Worker (if applicable) - Knowledge of ICD-9 and ICD-10 coding systems for medical documentation - Familiarity with medical terminology, particularly in the field of pediatrics - Experience in conducting data collection and analysis to inform treatment plans - Proficiency in motivational interviewing techniques and psychotherapy modalities - Strong interpersonal skills and ability to build rapport with diverse populations - Understanding of trauma-informed care principles, particularly in relation to PTSD - Excellent verbal and written communication skills Note: This job description is not intended to be all-inclusive.
