Job Description
Responsibilities:
- Conduct patient assessments to determine their needs and develop appropriate treatment plans
- Collaborate with healthcare professionals to coordinate patient care and ensure continuity of services
- Maintain accurate and up-to-date medical documentation and records
- Conduct diagnostic evaluations to assess patients' mental health conditions
- Implement evidence-based treatment modalities such as cognitive behavioral therapy
- Assist patients and families in end of life planning
-Provide bereavement and greif support
- Advocate for patients' rights and assist them in accessing necessary resources and support systems
Experience:
- Bachelor's or Master's degree in Social Work or related field
- Experience working in a home health or hospice setting is preferred
- Knowledge of special education laws and regulations
- Strong assessment skills to evaluate patients' mental health conditions
- Proficiency in conducting group therapy sessions
- Ability to work collaboratively with interdisciplinary teams
