Mountain View, a Nursing and Rehabilitation Center is currently seeking a Full Time Social Worker to join our compassionate team of caregivers.

Mountain View is a continuing care community that brings together short term rehabilitation, 24 hour long term nursing care, memory care neighborhood and medical services in the heart of the coal region. We believe in creating an environment and atmosphere that supports the well being of all our guests.

One year experience in social work preferred. This position is Monday through Friday with no weekends and paid holidays. Additionally you will earn paid time off and will be eligible for an excellent insurance package.

Position Description: Under the supervision of the Social Service Director, maintains the Social Service Department based on the needs of the type of residents care for in the facility, including the community agencies to provide economic, psychological, and medical assistance. This position provides medically-related social services to our residents, to attain or maintain the highest practical physical, mental, and psychosocial well-being of each resident.

Position Responsibilities

Specific responsibilities of the position include, but are not limited to, the following:

  • Meet/greet each new resident and their families. Assist with the admission process and throughout their stay to provide a comfortable experience.
  • Explain to resident/responsible parties, waiver, residents’ rights, services provided such as dental, podiatry, and eye care.
  • PASRR-Target identification and reporting to proper agencies.
  • Obtain Social Service history for the chart.
  • Coordinates care plan meetings, invitations to families/responsible parties, and residents.
  • Assessment completion and documentation, including care planning.
  • Addresses comfort care, hospice and end of life care with physicians, interdisciplinary team, resident and families/responsible parties.
  • Monitor mood and behaviors, psychotropic medications use, and follow-up as needed. Monitor behavior flow sheets, formulate behavior management plans as necessary. Arrange follow-up services for psychiatry, psychology, counseling, and therapy as needed.
  • Review all advanced directives and power of attorney for residents on admission and as needed.
  • Responsible for all grievances and missing items reports. Maintains records and reports to QA.
  • Resident discharges:

1. Transportation arrangements

2. Orders equipment and home health services

3. Logs all discharges, notifies all departments

4. Follow-up calls to discharge residents to ensure they are receiving the proper services.

  • Adheres to the established facility policies related to infection control and reducing the potential spread of infection.
  • Works cooperatively as part of the health care team to evaluate and revise current practices, policies and procedures.
  • Maintains and promotes relationships with residents, visitors, and other staff members, and government regulators by presenting a professional appearance and positive attitude.
  • Understands, complies with, and promotes all rules regarding residents’ rights and confidentiality.
  • Practices safety in provision of services, and promptly reports and hazards or violations to your supervisor.
  • Participate in committees of interest.
  • Performs additional functions or duties, asssigned by the Director of Social Services.

Essential Knowledge, Skills and Ability

Education/Certifications/Experience

  • EDUCATION: Degree in Social Work or Human Services field including but not limited to, Sociology, Special Education, Rehabilitation Counseling, and Psychology.
  • EXPERIENCE: One year of supervised social service work in a health care setting.

Knowledge

  • CUSTOMER AND PERSONAL SERVICE: Knowledge of principles and processes for providing excellent customer and personal services.
  • ENGLISH LANGUAGE: Must have the ability to read, write, and follow oral and written directions at a level necessary to accomplish the job.
  • PSYCHOLOGY: Recognizing and being sensitive to resident and family concerns, stressors during time of admission or financial difficulty.
  • SAFETY AND SECURITY: Knowledge of relevant equipment, policies, procedures, and strategies to promote effective local, state, or national security operations for the protection of staff and residents, data, and the Facility as a whole.
  • MATHEMATICS: Ability to perform basic mathematics.

Skills/Abilities

  • ACTIVE LISTENING: Ability to give full attention to what others are saying. Taking time to understand the points being made, and asking questions, if appropriate. Ability to hear and respond to resident call bells.
  • COMMUNICATION: Ability to communicate effectively across the organization. Includes the ability to read, comprehend, and interpret documents such as safety rules and procedure manuals and the aptitude to write routine reports and correspondence. Ability to speak effectively and be understood by others.
  • MONITORING: Assessing performance of self, other individuals, or the Facility to make improvements or take corrective action.
  • CRITICAL THINKING: Using logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems.
  • PROBLEM SENSITIVITY: Ability to tell when something is wrong or is likely to go wrong. It includes recognizing and solving problems.
  • ORGANIZING, PLANNING & PRIORITIZING WORK: Ability to develop plans in order to organize, set priorities, and accomplish daily duties.

  • DEPENDABILITY: Reliable, responsible, and dependable, while fulfilling obligations. Must be capable of maintaining regular attendance.
  • INTEGRITY: Ability to be an honest and ethical employee while exercising both patience and tact and complying with the Facility’s policies and procedures.
  • TECHNICAL: Ability to understand equipment function and operation such as lifting equipment, oxygen usage, and other medical equipment. Ability to use computer technology as trained.

Position Hours and Travel

  • Hours are Monday through Friday 8:30 am to 4:30 pm. Shifts may vary based on the needs of the facility.
  • Overtime hours, with little or no notice, although unlikely, may be required, if deemed necessary by Administration.
  • Although unlikely, local and out-of-town travel may be required for training purposes. Average travel expectation is less than 1%.
