Job Description
Title: Home Support Social Worker
Territory: Los Angeles County
Summary: Provides a full range of case management services to include psychosocial assessment of individuals, environmental assessment for safety, identification of potential needs, medication safety and reconciliation, short term treatment, referral, and crisis intervention.
Responsibilities:
· Conducts assessment/evaluation and diagnostic sessions for individuals including review of current medications. Provides referral, short term counseling, care coordination and crisis intervention services as appropriate. Submits reports to program as required by contract or mandated by law. Leads clients to development of strategies for conflict resolution.
· Reviews and evaluates client progress and establishes follow-up contact.
· Maintains confidential client records and documentation of service delivery.
· Documents activities in the required systems which can include both paper and electronically.
· Prepares reports on program activities as required.
· Manages the delivery of services to targeted program participants or beneficiaries.
· Interaction with staff in the health care delivery system.
Qualifications:
Minimum Education: Bachelors in Social Work or related field (psychology, gerontology) required.
Minimum Experience: 2 Years, combined education/experience as substitute for minimum experience.
Minimum Field of Expertise: Social work, home visits with all populations.
Additional Requirement:
Copy of current automobile insurance provided annually of $15,000/$30,000 for injury and $5,000 for damage to property.
