Social Work Care Manager for Parnters in Care

Health Projects Center (www.hpcn.org) is recruiting a Hybrid/RemoteSocial Work Care Manager to provide support to Health Projects Center’s Partners in Care Program. Reporting to the Associate Director, the Social Work Care Manager plays a pivotal role in providing essential support and case management to individuals with significant health needs. In this position, you will assume responsibility for evaluating participants, discerning functional and health limitations that hinder independent living. Collaborating closely with participants, the Social Worker will develop tailored systems and supportive strategies to ensure a safe and healthy home environment, fostering the ability for individuals to age in place within their community. About us

Health Projects Center is a 501(c)3 non-profit corporation established in 1980. We provide exceptional community-based programs that support elders to live and age with dignity at home. We deliver culturally appropriate programs in both English and Spanish for older adults, family caregivers and health workers.

Health Projects Center serves Monterey, San Benito, and Santa Cruz Counties. We run the Del Mar Caregiver Resource Center, Multipurpose Senior Services Program, and the Central Coast Area Health Education Center. Our programs support over 1,300 families annually to live and age at home at a fraction of the cost of an institution.

Duties

  • In-Depth Home Assessments:
    • Conduct comprehensive assessments covering psychosocial needs and environmental safety concerns within participants' homes, ensuring a thorough understanding of their unique circumstances.
  • Educational Support and Referrals:
    • Provide personalized education and offer referrals to local community resources, empowering participants to navigate challenges and continue living safely in their homes.
  • Service Coordination and Verification:
    • Take a hands-on approach in arranging, coordinating, and verifying services and interventions identified in participants' care plans, fostering a seamless and effective support system.
  • Record Documentation and Compliance:
    • Ensure meticulous and accurate documentation of participant records, adhering to program requirements and maintaining compliance standards.
  • Caseload Management:
    • Effectively manage a caseload of 25-40 clients per month, offering personalized attention and care to address their individual health and social support requirements.
    • Provide comprehensive financial counseling, assisting clients with personalized budgeting, money management strategies, debt reduction, and savings, empowering them to make informed financial decisions for enhanced overall well-being.
  • Continuous Monitoring and Care Plan Adherence:
    • Vigilantly monitor participants' needs and provisions outlined in the care plan, ensuring that the ongoing support aligns with their evolving health conditions and objectives.

Qualifications

  • Possession of a bachelor’s degree in Social Work, psychology, counseling, rehabilitation, gerontology, sociology, or a related field.
  • Exceptional organizational skills coupled with a demonstrated ability to meet deadlines effectively.
  • A positive and health-conscious attitude towards clients, our staff, and collaborative programs.
  • Willingness and ability to undertake regular travel within Santa Cruz and/or Monterey County.
  • Proficient computer skills with a comfort level in handling electronic records and charting.
  • Possession of a master’s degree in Social Work, psychology, counseling, rehabilitation, gerontology, sociology, or a related field, a plus
  • Hands-on experience in actively working with older adults, preferred.
  • Proven track record in delivering supportive services to older adults and individuals in need, preferred.
  • Preferred fluency in both English and Spanish.

Conditions of Employment

  • Possession of a valid California driver's license and access to an insured and reliable vehicle.
  • Must pass a criminal background check.
  • COVID-19 vaccination required.

Compensation and Benefits

  • Starting salary is $26.00-$28.00 per hour depending upon skills and experience.
  • We pride ourselves in maintaining a flexible and friendly work environment. We provide comprehensive benefits including paid vacation (15 days); paid sick leave (80 hours); company paid medical, dental, vision, chiropractor/acupuncture, and life insurance; travel reimbursement; educational/fitness allowance; and pre-tax retirement plan with 6% of salary contributed by employer.
