The Social Service Assistant at Immaculate Mary Center plays a crucial role in supporting the Social Services department and contributing to the overall well-being of our residents.
This position involves working closely with residents, their families, and the interdisciplinary team to ensure a holistic approach to care.
Responsibilities:
Resident Support:
Provide emotional and social support to residents, fostering a positive and inclusive community within the nursing home.
Assessment and Documentation:
Assist in conducting social assessments of residents and maintain accurate and up-to-date documentation of Social Service interventions.
Family Engagement:
Collaborate with residents' families to address concerns, provide information, and involve them in care planning to enhance the overall resident experience.
Care Planning:
Participate in care planning meetings, working closely with the interdisciplinary team to develop and implement individualized care plans that address the social and emotional needs of residents.
Advocacy:
Advocate for residents' rights and assist in resolving concerns or issues related to their well-being.
Crisis Intervention:
Provide support during times of crisis, working with residents and families to address emotional and social challenges.
Community Resources:
Connect residents with relevant community resources and services to enhance their overall quality of life.
Qualifications:
- Experience in Social Work or related field preferred.
- Previous experience in a healthcare or longterm care setting is advantageous.
- Strong interpersonal and communication skills.
- Empathy and compassion for the elderly population.
- Knowledge of relevant regulations and ethical standards in social work.
