Job Qualifications:
- Should have a minimum, one (1) year, social work experience in a health care setting working directly with individuals
- MEDICAID experience in an LTC/SNF setting preferred.
Responsibilities:
- Participate in discharge planning, development and implementation of social care plans and resident assessments.
- Interview residents and families to obtain social history.
- Work with community resources to ensure the patient has a safe plan when they transition out of the facility.
- Possess the ability to seek out new methods and principles and be willing to incorporate them into existing Social Services
