Description
St. Croix Health is seeking a temporary Social Services Assistant to serve in a position for 3 - 6 months on a part to full-time basis.
Job Summary:
The Social Services Assistant works under the Social Services Supervisor assisting Social Workers and Discharge Planners on providing patient services. This position will assist patients with identifying and obtaining available benefits and community services. In addition to initially connecting patients with benefits or services, this position is responsible may follow up with patients to ensure that they are receiving the intended services and that the services are meeting their needs. This position may offer advice, provide access to community services or governmental programs, and handle the paperwork related to these programs.
Essential Duties and Responsibilities:
- Assists social workers and discharge planners with daily tasks.
- Works with social workers to determine treatment plans for patients.
- Research services for patients at the time of discharge, such as internal services and programs, local, state, and regional public programs and non-profit services that may support patient's discharge and treatment plan.
- Helps connect patients and their families with internal and external support programs and resources. May include navigating program connections, facilitation of paperwork, registration completion, and coordination between resources or agencies.
- Checks in with patients in accordance with the treatment and discharge plan to ensure follow through in service support and connection.
- Assist patients with specific discharge needs such as housing, food, clothing, and/or transportation.
- Works with team to support development of internal workflows and coordination to improve patient's access to services and care needs.
- Follow patients through clinic, outpatient setting, inpatient setting, and upon discharge.
- Communicate with patients, care team, community resource staff and co-workers.
- Document in EMR chart and shadow chart of patients.
- Communicate and assist all clinic and hospital departments as needed.
- Adhere to NASW Code of Ethics and MPSW 20.

Education & Licensure:
- High school diploma required
- Associate's or bachelor's degree in social work, social sciences, human services, or related strongly preferred.
Experience:
- Experience in social work advantageous.
- Experience with relevant assessment tools and data entry software.
- Familiarity with NASW Code of Ethics and MPSW 20.
