JOB SUMMARY: This non-exempt management position, in cooperation with the Services Director, is responsible for ensuringpeople receiving waiver services in a residential and community setting are provided support which promotes dignity and respect; isindividualized according to the person's preferences, strengths and needs; and creates opportunities for meaningful relationships andcommunity involvement.This position supervises direct support professionals and provides service coordination for individualsliving in their own home.This position is also responsible for ensuring that policies, procedures, and practices comply withapplicable state and federal law and are aligned with Frontier's commitment to person-directed services.ESSENTIAL FUNCTIONS: SERVICE COORDINATION DUTIESo Provide case management assistance in collaboration with the Individual(s) reflecting the individual's choices andpreferences for the service being provided.Examples of this may include coordinating... support and access of medical,education and vocational services or obtaining documents such as a social security card.o Research Assisted Living Home licensing regulations and ensure the home maintains compliance with licensing ifapplicable.o Schedule, plan, and provide Day Habilitation and/or Behavioral Health services as outlined in the Individual's plan ofcare and bill for the service.o Ensure documentation of all significant events and activities are maintained in accordance with agency policy.o Convene and facilitate person-centered service team planning meetings.o Develop and update as needed information in Therap including service goals and objectives.o Monitor health and safety issues in the home, making necessary reports to appropriate person or persons, i.e. CriticalIncident Reports, Adult Protective Services Reports, etc.o Respond to individual and/or team member service-related concerns.o Participate in transfer meetings pertaining to newly referred individual(s).SUPERVISION DUTIES: o Supervise Direct Support Staff (DSPs) assigned to caseload.This includes coordinating interviews for new staff asneeded for Individuals on caseload.Engage in reflective supervision quarterly after initial 90-day probationary period.Provide weekly reflective supervision in the initial 90-day probationary period.o Ensure each staff has training and orientation directly related to the Individual and the specific services beingprovided as well as training on Global Hazard Systems and Safety Data Sheets.o Provide ongoing supervision, coaching and mentoring to DSPs to support quality service delivery.Developperformance improvement plans as needed.o Conduct timely performance appraisals, including assessing how the employee has performed and what they can do toimprove performance.o Respond to staff after hours, weekends, and holidays to resolve staffing issues and service concerns.o Review Therap entries including but not limited to service notes, T-logs and medication logs for quality andaccuracy.o Produce progress reports for care coordinators and other team members.ADDITIONAL REQUIRED JOB DUTIES: o Attend all required trainings and meetings.o Participate in agency-wide quality improvement, including developing ways to measure performance, collecting data onperformance and using the data to improve service delivery.o Conduct and oversee quarterly file reviews and monthly data reporting.o Other related duties as assignedQUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIRED: Bachelor's degree in psychology, social work, rehabilitation, nursing or a closely related human services field from an accreditedcollege or university, and one year full time of paid work experience with human service recipients and providers OR Two years ofcollege in psychology, social work, rehabilitation, nursing or a closely related human services fields from an accredited college oruniversity, and two years full time paid work experience with human service recipients and providers OR Four years of p
