Job description

Part-time 1-19 hours/wk. $19.22+DOE

In accordance with Assets, Inc. core values and mission, this position is responsible for developing Individual Plans (IP’s) for treatment and services through gathering information and conducting assessments, facilitating the team planning process, preparing written plan documents and evaluating plan implementation for served individuals experiencing mental illness and / or developmental disabilities.  Performs all functions in accordance with established company policies as well as departmental procedures and parameters. This position may require de-escalating verbally or physically aggressive individuals according to identified procedures and individual client protocols.

Essential Duties and Responsibilities
  1. Adheres to and demonstrates Assets, Inc. core values, mission, policies, and compliance plan in the performance of all aspects of this position.
  2. Facilitates a team process to develop individual plans for treatment and services to meet Assets, Mental Health Medicaid, Home and Community Based Waiver, and Developmental Disabilities Grant requirements (Service Planner will specialize in one of these funding sources), including mandated time frames. This includes, but is not limited to the following:
  • Facilitating the individual’s expression of his / her hopes and dreams, and translating them into written plan goals that are meaningful to the individual.
  • Collecting and incorporating information from professional evaluations and other pertinent records into the planning process (e.g., comprehensive assessments, psychological evaluations, incident reports, progress notes and hospitalization records).
  • Ensuring that the served individual and his / her support team are fully involved and engaged in the planning process.
  • Drafting recommended goals and objectives for treatment and services, intervention strategies, tracking forms and other plan package components based on the desires of the individual, as well as the input of the treatment team.
  1. Facilitates formal team review and approval of the Individual Plan. This includes, but is not limited to the following:
  • Producing and compiling the final plan package
  • Gathering signatures upon Team Coordinator approval
  • Filing and distributing the IP and associated documents.
  1. Mentors Life Coaches, Job Coaches and Care Providers on the planning process and plan implementation.
  2. Educates staff on the importance of developing accurate and informative tracking records. Provides training on how to complete tracking forms.
  3. Evaluates the effectiveness and relevance of Assets services to the individual in light of their personal goals and needs, and recommends service adjustments through the plan review process.
  4. Regularly communicates progress made over the last planning period and new information relevant to treatment / service decisions to the treatment team.
  5. Provides professional and tactful feedback to Life Coaches and / or Team Coordinators regarding staff performance and compliance with tracking requirements.
  6. Completes required training and participates in on-going professional training.
  7. Attends team, department, and Assets meetings as requested.
  8. Performs other tasks assigned by supervisory personnel.

 

Minimum Qualifications

Education and Experience:  

  • High School Diploma or GED required.
  • Associate Degree (A.A.) in human services or related field;
  • OR two (2) years experience in the human services field (case management, direct services, etc.);
  • OR equivalent combination of education/training and experience.

Language Skills:    Excellent professional writing skills.  Ability to write routine correspondence.  Ability to speak effectively before groups of people receiving services, employees or the organization.  Ability to listen well to others and to capture their sentiments in writing, to facilitate team discussions resulting in consensus.

Ability to read and interpret professional documents, such as psychological and psychiatric assessments, social history reports, hospitalization records, regulations and procedure manuals. 

Mathematical Skills:     Ability to add, subtract, multiply and divide in all units of measure, using whole numbers, common fractions, and decimals.  Ability to compute rate, ratio, and percent and to draw and interpret bar graphs.  Ability to interpret basic statistics, e.g., mean and standard deviation.

Reasoning Ability:    Ability to solve practical problems.  Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to contribute to the development of creative solutions as a team member.

Certificates, Licenses, Registrations:    Must have an insured vehicle and current Alaska driver’s license (with no or low points) or reliable transportation.  Must be able to pass a criminal background check under the state regulations.

  6. Ability to develop useful and useable data tracking tools.
  7. Ability to combine data with secondary information sources (e.g., progress notes and oral accounts), and to evaluate individuals' progress toward achieving goals and gaining independence.
  8. Ability to evaluate the need for and adequacy of services through conducting functional assessments, consulting with others and using secondary sources.
  9. Ability to communicate with a diverse group of people and to involve them in the planning process.
  10. Ability to facilitate team meetings, e-mail and other means of team discussion.
  11. Ability to communicate and work with individuals experiencing developmental disabilities and/or mental illnesses.
  12. Competent use of e-mail, MS Word, MS Excel, and e-file management.
  13. Open to supervision and feedback from others.
