Job Description:
This Service Coordinator position will provide various duties in support of grants associated with the Duke Infectious Diseases/HIV Clinic. This person in the position will report directly to Dr. Mehri McKellar.
Job duties include:
10% Maintain all Policies and Procedures of Duke University Hospital, Ryan White Part B, and North Carolina Department of Health and Human Services.
10% Identify, coordinate and facilitate client referrals for treatment resources. Refer clients to community service providers as appropriate.
25% Execute time-sensitive process of bi-annual HIV Medication Assistance Programs (HMAP) recertification including completing and processing applications for patients. Maintain and update Maestro Care and CareWare.
5% Verify the median incomes levels are being met to ensure eligibility for assistance.
10% Complete and approve medication financial assistance requests
10% Client record-keeping and reporting
5% Provide excellent customer service.
10% Collaborate and build good working relationships with HMAP, Walgreens, and clinic staff
10% Serve on a multi-disciplinary team with clinical social workers, pharmacists, medical providers, and community case managers and agencies
5% Special projects and tasks as needed or required by grant funder or clinic staff

Ability to maintain the highest level of confidentiality and use excellent judgement in working with clients.
