Job Description
- The Resident Care Coordinator, also known as the MDS Nurse Coordinator, is an experienced healthcare professional responsible for ensuring current care plans and MDS (Minimum Data Set) for all residents.
- This position ensures all documents related to the plan of care are consistent with current Federal, State, and local standards, guidelines, regulations, and facility policies as directed by the Administrator.
- Review department policies, procedures, and job descriptions annually for revisions and make recommendations to the Administrator.
- Oversight of scheduling, completion, and accuracy of all MDS item sets.
- Coordinate development of a written plan of care for each resident, ensuring realistic and measurable goals.
- Chair Resident Care plan conferences and relay concerns to the Administrator.
- Ensure residents/families/responsible parties are contacted for care plan conferences per RAI guidelines.
- Plan, conduct, and participate in In-service Education Programs related to the RAI process.
- Assist in implementing and maintaining an effective orientation program for new employees.
- Monitor residents' medical records for consistency and content, addressing discrepancies.
- Maintain ICD-9 codes for current diagnoses.
- Report IT concerns to the help desk in a timely manner.
- Participate in the development, maintenance, and implementation of the facility’s QAPI program.
- Attend relevant meetings as directed by the Administrator.
- Perform other related duties as assigned.
- Current, unencumbered RN or LPN license in the state.
- Current CPR certification or ability to obtain within 90 days of employment.
- Working knowledge of OBRA, IDPH, IDPA (for Illinois homes), and Wisconsin DQA (for Wisconsin homes) standards.
- Proficient in English language (reading, writing, speaking).
- Ability to communicate tactfully with residents, family members, visitors, personnel, government agencies/personnel, and the general public.
