Title: RN COORDINATOR STROKE PROGRAM
Scheduled Hours:
- Day Shift; Monday-Friday hours
- Part time (24-hour work week); 2- 12 hour shifts a week
- Ability to take call as needed
Job Summary:
This position coordinates and facilitates the stroke population for the purpose of optimizing the stroke patient's health status, utilizing holistic resources available within the system. The Stroke Coordinator renders highly professional and technical direct or indirect patient care requiring evaluative judgment, specialized knowledge, and professional skills. The individual in this position collaborates with physicians and the multidisciplinary team to ensure the highest standard of care. Participate/assist in data collection and analyses regarding stroke population.
- Collaboratively develops and implements disease management protocols/guidelines that reflect the most current research and practices in the stroke patient.
- Ensures education for patients, families, and members of the healthcare team is provided and relevant.
- Coordinates stroke care with patients and families during the inpatient episode.
- Coordinates performance improvement efforts for the stroke program.
- Maintains clinical and professional skills necessary to provide services to patients. organizations and continuing education.
- Assists, coordinates, and participates in Stroke Support groups. Development of Stroke support groups as needed.
- Upholds the highest degree of customer service as outlined in the Standards of Behavior expectations.
- Facilitates continuity of care by acting as a liaison with other professionals based on individualized patient concerns or needs.
- Identifies grant and clinical research opportunities for the Stroke Program.
- Coordinates strategies to ensure quality, efficacy and cost-effectiveness for the stroke patient across multiple setting
Position Requirements:
- Bachelor’s Degree in Nursing.
- Graduate of an accredited school of nursing
- Illinois RN license maintained
- 3 – 5 years focused nursing experience; preferably with the stroke population
- CPR certification, ACLS certification
- Certification in specialty within 2 years of hire
- Strong leadership skills
- Excellent communication skills
- Ability to work collaboratively with a multidisciplinary team
- Computer skills including data entry and report generation necessary
- Required to work weekends, holidays, and off shifts as needed.
- Travel to alternative care delivery sites is required.
- Ability to lift up to 35 pounds without assistance. For patient lifts of over 35 pounds, or when patient is unable to assist with the lift, patient handling equipment is expected to be used, with at least one other associate, when available. Unique patient lifting/movement situations will be assessed on a case-by-case basis.
