SalaryTotal Family Healthcare is currently accepting resumes for experienced RN and LPN candidates with a background or interest in care management. The position will be in a family practice environment, working with providers to create care management plans with patients. Responsibilities will include creating care plans, coordinating outpatient care, telephone follow up, and other clerical and clinical tasks related to that patient population. Interested candidates should be highly organized, very compassionate, and have a strong ability to self manage their time.
