Overview
Are you a skilled and compassionate Registered Nurse passionate about long-term care? Hillview Health Care Center seeks anMDS Coordinatorto join our dedicated team.
As an MDS Coordinator, you will play a vital role in ensuring the timely and accurate completion of the MDS (Minimum Data Set) for nursing home resident assessment and care planning. You will work collaboratively with the Director of Nursing and interdisciplinary teams to facilitate integrated care plans and maximize reimbursement for the facility.

Location: Hillview Health Care Center is in La Crosse, WI, on the city bus line and bike trails!
Shift: 8 AM to 4:30 PM, Monday through Friday!
  • 60-80 hours per pay period (two weeks).
  • On-Call every fourth weekend.
Wage depends on experience!
- Up to $42.18/hr on the Supervising Registered Nurse scale.

ESSENTIAL JOB FUNCTIONS
Role models expert nursing skills in assessment, planning, implementation and evaluation of nursing interventions for facility residents. Responsible for the accurate and timely completion of the MDS, as required by OBRA/Federal regulations and PPS (Prospective Payment System) and insurance companies that require RUGS scores for payment.
Coordinates and/or participates in care planning process, especially for residents with complex care needs. Provides MDS schedule daily and monthly logs.
Sets up MDS/ tracking/ nurse assessments needed for appropriate documentation and evaluations for each assessment.
Completes admission careplan for new residents and updates plan of care with hall nurses as needed.
Participates in chart review and record request for medicare appeals.
Participates in ongoing evaluation of current clinical practice and identifies measures to improve care.
Provides counseling for residents and family, based on care and clinical practice. Assess and starts self medication programs. Attends care conferences.
Demonstrates knowledge and clinical competence in area of resident care standards. Attends Medicare meetings.
Works with ECS as needed for MDS related issues, nursing and charting issues.
Assesses staff learning needs and interests identified from clinical observation and participation. Provides education to nursing staff related to the thorough and accurate completion of the MDS coding/documentation to assure maximum reimbursement structure.
Sits on various internal committees (i.e. Falls, QAPI). Facilitates MDS audits and research. Assists with survey related issues/audits in collaboration with Nursing Administration.
Keep current on MDS changes/updates through Leading Age, AANAC and other sources.
Provides continuing education to the team members as the MDS and RAI manual are revised.
Develops, teaches, assess and updates restorative nursing programs to meet the needs of the residents. Consults with therapy staff, as needed.
Coordinates completion and documentation of restorative programs with nursing staff.
Facilitates communication between departments to promote improved comprehensive care for residents, within our reimbursement structure.
Review charting and provide education and re-education.
Works safely and follows safety practices and standards of the facility; responsible for reporting or correcting any existing or potential safety or accident hazard.
Maintains resident's rights and confidentiality of resident information; reports all suspected cases of resident abuse to the Administrator.
Employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness.
In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and clients. Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.
Physical demands: Sits at desk, uses computer, uses hearing, near vision, fingering for keyboarding, reading and writing. Handles papers and manuals, etc., lift files, copy paper and other office materials. Sits, stands, bends, and reaches intermittently throughout the day. Occasional activities include stooping, crouching, pushing carts. May lift up to 35 pounds, transfer up to 50 pounds. Uses nursing-related equipment.
RELATED JOB FUCTIONS
May fill in as Building Supervisor and May fill in on the floor in any nursing capacity including CNA, charge nurse
May be scheduled on-call.
May work in specialty capacity, i.e. wound care, as assigned.
Supports all aspects of person centered care.

Knowledge, Skills and Abilities
  • Thorough understanding and working knowledge of the PPS (Prospective Payment System), the current MDS system and RUGS systems.
  • Is an extension of nursing management therefore, is expected to have excellent attendance, professional, respectful communication at all times and have the ability to schedule hours to meet needs of time sensitive data.
  • Knowledge of how various insurance companies reimburse us and provide RUGS scores accordingly.
  • Comprehensive knowledge of Medicare regulations. Thorough knowledge of application of the nursing process, assessment, planning, implementing and evaluating, completing accurate assessments and MDS entries that will result in correct resident assignment to the proper HIPPAs code and case mix state index to ensure the appropriate payment to the facility.
  • Skills in problem solving, negotiation and group leadership.
  • Ability to communicate effectively and establish and maintain good working relationships with other staff members, other health care professionals, employees, residents and guests.

Training, Experience and Other Requirements
  • Licensure as Registered Nurse and two years clinical experience in geriatrics, with emphasis on Medicare utilization, and reimbursement issues.
  • Preference for persons with experience with MDS (Minimum Data Set) duties.
  • May consider other relevant combinations of education and experience that provide equivalent knowledge, skills and abilities.
  • Be able to plan, organize, prioritize, work independently and meet deadlines.
  • CPR certified.
  • Must pass criminal background check as required by Wisconsin Caregiver law.
