Sage Group HealthCare Staffing is seeking Registered Nurse Home Health for a 13-week travel / contract position in Columbus, OH. Accepting both local and traveler candidates.

Pay Rate: Range $2000 to $2280 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.50/mile for distance covered between patient home appointments
Unit: Home Health
Shift:8 Hours; Days, 8.00a - 5.00p
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri, Must meet 32 productivity points per week
Position Requirements:
- Registered Nurse: 2 years - Required
- Recent/Current Home Health Nursing experience: 1 year – Required
- Start of Care (SOC) experience - Required
- OASIS experience: 1 year - Required
- Home Care Home Base (HCHB) charting experience – Highly preferred
- Must be Covid Vaccinated or provide medical or religious exemption
Education, License and Certification Requirements:
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- RN License for State of Ohio or Compact - Required
- BLS Certification from AHA or American Red Cross - Required
Job Description:
- Provide and coordinate care for assigned Home Health patients
- Complete Start of Care (SOC) and admissions for the assigned case load
- Develop and implement nursing care plans for patients as assigned.
- Empower patients and families to reach healthcare goals by educating them on disease management.
- Provide Home Health nursing care and services within the standards and scope of nursing practice as defined within the state Nurse Practice Act regulations, policies, procedures, and protocols, as well as facility and corporate policies and procedures.
- Ensure physicians’ orders are legible if taken verbally and transcribed appropriately per policy and procedure.
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status.
- Document and complete charting
- Territories: Columbus and surrounding areas
